  2. History  Based on the “Volunteers in Medicine” model which originated in Hilton Head, South Carolina.  Community ownership and passion instrumental in the creation of CVIM.  The doors of CVIM opened in February of 2003.

  3. Vision “All Centre County residents have access to healthcare.”

  4. Mission “ Centre Volunteers in Medicine will serve and advocate for the medically underserved residents of Centre County.”

  5. Centre County By the Numbers  More than 11,000 medically uninsured  More than 33,000 estimated dentally uninsured Many earn to much to qualify for Medicaid yet not enough to afford medical coverage.

  6. The truth of the matter  Misconception  CVIM receives most of its funding through state dollars.  Misconception  People who use CVIM services do not work .  Misconception  If someone is sick, they’ll seek help.

Realities Why Patients Delay Seeking Care Inability to afford medication Shame & embarrassment with the deteriorating state of their condition Sharing of medication or insulin with a family member Fear of being diagnosed with a serious illness or surgery Fear of financial devastation due to lack of disability coverage Too proud to seek “charity” help Down playing illness due to perceived cost Fear of who would care for children

  8. Patient Eligibility Criteria Must reside in Centre County Family income at or below 200% of the federal poverty level No health insurance • To obtain free medical services No dental insurance • To obtain free dental services

Medical Services  CVIM provides:  Appointment with a family doctor:  Preventative Care  Urgent Care (illness)  Chronic Disease Management  Diagnostic Tests and Labs  Referrals to Specialists  CVIM Does not provide:  Surgery, emergency treatment, prenatal care, family planning services.

  11. CVIM Medical Clinic  Most commonly treated conditions include:  Hypertension  Diabetes  Anxiety  High Cholesterol  Depression

  12. Dr. Richard M. Madore Dental Clinic  Most commonly treated dental health needs include:  Emergency Evaluations  Extractions  Restorative Work  Dental Hygiene  Give Kids A Smile and Vision Day

  13. CVIM Case Management  Assist with enrollment applications for public assistance programs  Basic Needs Case Management  Oversee Tobacco Cessation Program

  14. CVIM Medication Assistance Program  MAP provides critical support to CVIM patients by offering prescription medications at no cost.  Donated through Indigent Drug Programs  Purchased at reduced cost  In 2013/2014 CVIM received over $1.2 million worth of medications for our patients.

  17. CVIM By the Numbers: Volunteers Professional Volunteers • 11 Dental Providers • 19 Medical Providers • 9 Licensed Pharmacists • Numerous Specialists 186 volunteers donated 16,974 hours of their time to the clinic in fiscal year 2013-2014

Volunteer Expectations  Fulfill the minimum hours requirement fro assigned position.  Report a few minutes early for your shift.  If you cannot fulfill your shift, give ample notice.  Notify the VC of a leave of absence ASAP.  Wear your nametag.  Keep accurate records of volunteer records.

  20. Confidentiality  Confidentiality Agreement  Under no circumstances should volunteers disclose any CVIM confidential information including:  Patient information  Donor information  Other clinic business

Student Volunteers  Must be at least 18 years of age.  Commit to a weekly shift for at least one full semester.  Provide reliable transportation  Observe the dress code  We do not offer shadowing opportunities  If you need a reference or recommendation, you must volunteer at least 40 hours.

  22. Clinical Volunteers  Clinicians can volunteer as little as 4 hours per month.  Professional Liability Coverage is available at no charge to all licensed providers .  Clinicians set the pace and number of patients seen.  Staff nurse is on site during clinic hours.

  23. Greeter  Position Description:  Greets patients/visitors at the door.  Helps patients sign in.  Assists with children while parents receive care.  Notifies clinic of patient arrivals  Time requirement:  One 4 hour shift per week, 8:30-1:00 or 1:00-4:30

  24. Receptionist  Position Description  Answer phones and direct phone calls  Greet and assist patients/visitors  Schedule appointments by computer  Screen clients for eligibility  Time Requirement  One 4 hour shift per week, 8:30-1:00 or 1:00-4:30

  25. Medical Records  Position Description:  Prepare charts for medical clinic.  File documents in charts.  Assemble chart for new patients.  Time Requirement:  One 2-3 hour shift per week, flexible scheduling

  26. Medication Assistance Program  Position Description:  Complete applications to IDP programs  Application follow-up  Time Requirement:  One 4 hour shift per week, Monday, Wednesday, or Friday morning

  27. Case Manager/Social Worker  Position Description:  Conduct needs assessments.  Make appropriate referrals.  Assist with applications for insurance PRIOR CASE MANAGEMENT EXPERIENCE IS REQUIRED  Time Requirement:  One 4 hour shift per week, scheduled as needed.

  28. Occasional Volunteer Opportunities  Special Events  Mailings  Fundraising Events  Projects  Work Days  Committees  Driver/lunch assistant  Time Requirement:  Flexible and occasional hours based on project or event
