  3. Working Links: accreditations We were recently awarded an ‘Excellent’ Merlin Standard which recognises and promotes sustainable excellence within supply chains. In July 2016 we were awarded a ‘Strong’ Provider Assurance Team (PAT) score, that covers governance, risk management and control arrangements. We also have continued certification with the Matrix quality standard for information, advice and guidance services.

Eligibility Criteria To be eligible for this Provision individuals must be unemployed or inactive, Work Programme Leavers and one of the following: � Unemployed after completing Work Programme � Health condition and/or Disability lasting more than 12 months including Physical, Mental Health and Behavioural issues � Recovering from Drug or Alcohol abuse It is expected that LEP Priority Group Participants will mainly fall into one of the following groups: � ESA Claimants in the Work Related Activity Group � JSA Claimants with a health condition � JSA or ESA ex-Incapacity Benefit Claimants � Universal Credit Claimants who are ex JSA or ESA claimants ESF Provision must be part-time for all Participants. The maximum duration of this ESF Provision for a Participant is a continuous period of 52 weeks from the Participant’s start.

Health & Wellbeing Improving client outcomes with health and wellbeing support � Health & Wellbeing model - Physical and mental health support from in-house Health Professionals � Identification of health related barriers to employment � Outcome focused - Improved health, empowerment, return to work and sustainability � Signposting – Linking with GP’s, Health Professionals and established local services to address customers health conditions or drug and alcohol misuse � Two levels of support : � General lifestyle management – confidence, motivation, weight management, sleep & routine, nutrition, smoking � Specific health related – physical (musculoskeletal, cardiopulmonary, neurological) and mental health issues (depression, anxiety, stress)

Our Delivery Partners Hubs in Durham City, Stanley, Bishop Auckland and Newton Aycliffe Engagement and guidance with focus on priority customers requiring Mental and Physical health interventions. Co Durham – Seaham and Peterlee End to end support for customers with multiple barriers including Health, Housing issues, Debt and Substance Abuse. Pan CPA – services reach across the region in locally based community venues. End to end support for disabled customers and specialist knowledge of barriers facing veterans. Working Pan CPA – Sites include Team Valley, Newcastle, Wallsend and Chester le Street Supporting customers recovering from substance abuse and women who have experienced domestic violence or offenders .

  13. Our Delivery Partners 4 sites around Gateshead Community based end to end support including money advice, housing support, skills training and work placements. Established site in Pallion, Sunderland Individualised customer focus with skills training and signposting to local support services. Pan CPA – 13 sports and community venues across Northumberland, Tyne & Wear and Co Durham Engaging customers through physical activity and mentoring into employment.
