JOB SITE SAFETY PROGRAM • Safety will be the overriding priority on this project. • The Contractor is responsible for administering its own safety program and shall comply with all on site safety, OSHA and local safety regulations. • The Contractor is responsible for the safety of all its employees as well as all its subcontractors. • It is the Contractor’s responsibility to provide orientation to all his employees and subcontractors. • Copies of orientation for each employee will be provided to the Construction Manager. • The Site Specific Safety & Health program for the Contractor and its Subcontractor must be submitted within ten (10) working days of contract award AND prior to start of work. • The Site Specific Safety & Health Plan is to include a list of emergency contacts. • M.S.D.S. chemical approvals will be required for all materials brought on site. M.S.D.S approval requests using the standard GM form and associated back- up must be submitted AND approved prior to bringing materials on site. This process is a minimum of ten (10) working days. 5
JOB SITE SAFETY PROGRAM (Cont.) • Personal protective equipment must be worn at all times. At a minimum, this includes the following: Hard hats o Safety glasses with side shields o Closed toed, over-the-ankle boots with rubber soles o • Cleaning of site and daily housekeeping is required. • Daily documented safety inspections will be performed on all equipment such as lifts, fork lifts, etc. to ensure the equipment and utilization practices are in compliance with GM standards. • Weekly safety walkthroughs will be led by the Contractor and the Owner will participate. • Accident, illness, injury, and incident investigation reports shall be submitted within 24 hours of incident or 8 hours of fatality and on the GM standard forms. • Project OSHA 300 Log providing man hours and safety incident summary shall be submitted by the 5 th of each month and on GM standard forms. 6
JOB SITE SAFETY PROGRAM (Cont.) • MIOSHA regulations and standard GM Safety policies will be followed and enforced throughout this project. • There is a “No Smoking” Policy. • No cameras or cell phones that can take pictures are allowed without prior authorization. 7
PRE-TASK PLANS • A list of the tasks requiring a Pre-Task Plan (PTP) must be submitted with the Job Site Safety Plan. PTPs will be required for all construction activities to include day-to-day and major activities. • All PTPs are to be site and task specific. “Boiler plate” PTPs will not be accepted. • PTPs associated with a general work activity shall be submitted at least (3) working days prior to the start of work. • PTPs associated with a shutdown request shall be submitted at least (10) working days prior to the anticipated shutdown. • PTPs associated with major activities or shutdowns may be reviewed at a separate meeting to ensure that coordination and safety have been thoroughly addressed with the Owner and affected parties at least (10) working days prior to the activity. • Due to the site being both an existing facility and occupied, the Contractor must include in the PTPs how the protection of GM property and personnel will be handled. • Use and protection of existing facilities is to comply with GM Construction General Terms and Conditions, Article 9.2. 8
MUST SAFETY AND DRUG PROGRAM • All employees assigned to GM Project sites in Michigan must be enrolled in the MUST Drug Alcohol Screening Program and Safety Awareness Training System. (See Attachment A and GM Construction General Terms and Conditions of the bid documents) • All Contractor and Subcontractor supervision, on-site management and craft workers must have (18) modules completed. • Documentation must be submitted prior to start of work verifying every on-site personnel is up-to-date. • Personnel working on the Warren Technical Center site more than 10 working days, must be registered to the Warren Tech Center MUST Site 144. 9
FIRE PROTECTION MEASURES • The Contractor shall be responsible to take all necessary actions and precautions to eliminate fire hazards and assure good housekeeping practices. • Hot work permits required for all welding, torching cutting or grinding operations performed on the job site. • Hot work permits must be obtained on a daily basis from G4S. Permits obtained for one shift is not valid for the next shift. • A designated person must remain on fire watch 30 minutes after cutting/torching operations have ceased until the area is secured and safe. • The Contractor is responsible for providing fire extinguishers throughout the construction site in accordance with GM Fire Prevention Standards. • No gas powered equipment will be permitted within the building. If this type of equipment is required, it must be fueled with propane, have scrubbers, and air quality monitoring is required during operation. • All staging areas shall be inspected and approved by Owner’s Fire Marshall. 10
ENVIRONMENTAL AND WASTE MANAGEMENT • GM will provide the appropriate disposal gondolas or containers for all general construction debris, asbestos-containing debris (regulated and non-regulated), flushing agents, disposed fuel, PCB containing ballasts, etc. • Cost for and transportation of all waste materials to a GM designated landfill or disposal site will be provided by the Owner . • The Contractor and it’s Subcontractors are responsible for the handling and sorting of the materials into the appropriate containers. Any waste streams not properly sorted will be re-sorted by the Contractor at no additional cost to the Owner . • The Contractor shall contact MPS to order the roll-off boxes or other necessary disposal containers in advance of the scheduled work and specify the designated placement. 11

SCOPE OF WORK The VEL Clean Room and Materials Lab Project includes the construction of a new coating room, cell assembly area and gowning room within the VEL Lab Area. The construction consists of new structural framing system and architectural, mechanical, electrical and life safety system upgrades as well as laboratory equipment installation as reflected on the Issued for Bid documents dated 12/17/13. GENERAL • Install and maintain temporary fire retardant construction enclosure barrier as necessary around the work area. • The Owner will occupy the VEL Lab Area, as well as the RSB, immediately adjacent to the Work area throughout the duration of this project. General tenant access must be maintained in the adjacent lab spaces and any disruptions to the occupied areas must be scheduled and coordinated in advance of the disruption. • Install and maintain temporary protection in the adjacent occupied buildings which become exposed to weather elements as a result of this work. 13
