Vision — key things to remember • Benefits run on a plan year basis (Oct. 1 – Sept. 30) • Members may see any licensed ophthalmologist, optometrist or optician • You will receive Moda’s discounted rate if you choose to see a Moda contracted provider Use Find Care, Moda’s online directory, to find one near you • • Your vision plan is not tied to your medical plan’s network
