  2. 2018 Open Enrollment Details • This will be a PASSIVE ENROLLMENT • Enrollment MUST be done ON-LINE using the ADP website • All benefits eligible employees must ENROLL ONLINE whether making changes to your current coverage or not • Benefit elections made during OPEN ENROLLMENT go into effect January 1, 2018 • After open enrollment CLOSES you will not be eligible to enroll or make changes to your coverage until the next open enrollment period……….unless you have a Qualifying Life Event (QLE )

  3. Types of Enrollment • Initial Enrollment - Within 31 days of date of hire • Qualifying Life Event (QLE) - When life changing event occurs • Open Enrollment - Once a Year

Qualifying Life Events What Counts as a Qualifying Event? • Getting married or divorced • Having a baby or adopting a child • Death in family • Change in your spouse’s work status • Change in eligibility for Medicare/Medicaid Supporting Documentation will be required to support the Life Event! You have 31 days from the Life Event to enroll!

  5. Initial Enrollment Waiting Period • Priests and Religious: - First day of the month following date of hire • Principals, Assistant Principals, and Teachers under contract: – First Day of the month of date of hire • Lay Employees: - First day of the month following 60 DAYS of employment

  16. 2018 Basic Life and Accidental Death and Dismemberment • Basic Life and AD&D coverages are provided at no cost to all benefits eligible employees through The Hartford Life Insurance Company. - One-and-a-half times base annual salary, rounded to the next thousand* $50,000 maximum Reminder: It is always a good idea to update your Beneficiary or Beneficiaries. Forms are available on the Archdiocese website .

  17. 2018 Supplemental Life Insurance You have the option to purchase additional life insurance for you and your dependent children* • Employee : - $25,000 increments up to $100,000 - Not to exceed 5X basic annual salary - $100,000 Guaranteed Issue up through age 64* - Evidence of Insurability May Be Required • Children: - $10,000 per child for children up to age 26 - $500 for a child under 6 months Spouse Coverage was discontinued as of January 1, 2016. If you elected spouse coverage prior to 2016, the coverage was grandfathered. You will not be able to increase/decrease the coverage but can elect to drop the coverage. This service is also administered by The Hartford Life Insurance Company

  18. 2018 Disability Benefits • The Archdiocese of Galveston-Houston offers Short-Term Disability (STD) and Long Term Disability (LTD) benefits to all benefits eligible employees • The Hartford Insurance Company is our current insurance carrier and manages all STD and LTD claims • In addition to managing STD and LTD, The Hartford also manages applications and makes determinations for employees who apply for the Family Medical Leave Act (FMLA) • WHEN ANY EMPLOYEE TRANSITIONS TO LONG TERM DISABILTY, THE EMPLOYEE WILL BE EXITED FROM EMPLOYMENT AND THE ARCHDIOCESE OF GALVESTON- HOUSTON’S HEALTH AND DENTAL BENEFITS WILL END ON THE DATE LTD ELIGIBILITY BEGINS

  20. 403(b) Retirement Savings Plan • Archdiocese sponsors two 403(b) accounts to help you save for retirement: -VALIC and Fidelity Investments - Local representatives are available to assist with enrollment, investment choices, and to answer any questions you may have • 403(b) is funded by your pre-tax contributions through payroll deductions • The maximum annual deferral contributions will go up by $500:  2017 - $18,000  2018 - $18,500 • The annual catch-up provision for employees over the age of 50 for 2017 and 2018 will remain the same at $6,000
