You May Continue Your Coverage • When coverage is effective and would otherwise terminate because you end employment with your employer, coverage may be continued. • You may continue the coverage that is in force on the date your employment ends, including dependent coverage that is in effect. • Coverage may not be continued if you fail to pay any required premium. • Premium for continued coverage is paid directly by you. 5
