Absence Reporting Please call 908-479-6336 option 1 Excused absence = notify office MORE than 3 days = physician’s statement Submit physician authorized excuses and absence excuses to the Health Office Without documentation = recorded as unexcused Ten unexcused = referred to the municipal court No PE = No after-school sports participation No School = No after school activities
Medication Administration All medications including over the counter must be given by the school nurse Signed permission from the parent/ guardian in addition to the physician ’ s order must be completed The medication must be in the original labeled container
