Strengthening Primary Care Delivery of the GP 5 Year Forward View • Care navigation roles • Investment in HCA training and accreditation across the city • Workflow Optimisation training - 54 members of staff trained across 15 practices. • eConsult rolled out to practices Supporting primary care network development
Integrated Primary Care Service 96.3% compliance when measuring time to treat performance 100% shift fill rate for clinical roles ED attendances have reduced by -1%, during the period of operation Clinical assessment of NHS 111 ED dispositions has diverted 86% from attending
Enhanced Care Home Team Currently active in 5 homes across the City SCAS conveyances reduced by 31.8% A&E attendances reduced by 31.8% A&E admissions reduced by 8.5% 72% reduction in calls from care homes to GP Practices 75% reduction in urgent GP visits to the care homes Roll-out to 5 more homes during 2019-2020
Collaboration with the Voluntary and Community Sector:
A new service to support resilience and mental wellbeing in a welcoming, safe space providing emotional & practical support under one roof. A pathway of wellbeing support alongside The HIVE and Social Prescribing. Work with individuals to identify the cause of their distress and put together a bundle of support Any adult (18+) in Portsmouth will be able to access Positive Minds; there is no need to see a GP, no need for a referral or to be known to health or care services in Portsmouth.
Integrated Community Team Action based learning approach to develop an integrated operational assessment and care planning process for community health and care services. Reconfiguring SystmOne to reflect the new case management process. The first locality (South) will be scaled-up incrementally
So what comes next…? • Stronger partnership with Portsmouth City Council • Further integration of community teams aligned with primary care networks • Strengthening urgent and intermediate care services in the community • Closer alignment of delivery models across Portsmouth and South East Hampshire
