Flu Clinics Autumn 2015 Clinics will be held at the surgery in September and October on a variety of days and times to give patients a choice of when to attend. This year clinics will again be ‘by appointment’ which means that patients will be given a time to arrive for their flu vaccination, thus reducing waiting times for patients and ensuring the smooth running of the clinics in the surgery.
Booking your appointment for flu vaccine If you are eligible for the flu vaccine please call in or ring the surgery on 01743 891401 to make your appointment for your flu vaccination. If you are unsure if you are eligible please ask one of the reception staff who will be able to advise you.
Collecting Prescriptions It is very important when collecting medication for yourself or someone else that the prescription is filled in correctly. If a false claim of exemption is made a penalty charge may be issued and the patient could be prosecuted. Routine checks are made.
Completing prescription forms Please ensure that when filling in the back of the prescription you: • tick the relevant box whether you are the patient or representative • sign in the bold box • if the representative then print name and address underneath. • date the prescription
Prescription exemption If the patient has an exemption and does not pay for their prescriptions then please make sure you know what the exemption is and also bring the exemption card with you. If you do not have proof of the exemption then we have to mark the ‘evidence not seen’ section which will be picked up by prescription services.
GP Appointments Making the most out of your appointment We want to make sure that you choose the best option when you see the GP, so you get the most out of your appointment. There are different ways of seeing a GP so please see the next couple of slides to decide which appointment would be best for you.
Open Clinics The walk in surgery is available Monday to Friday. You can book in between 9-10am to see a GP the same morning. Open surgery can get very busy which means there may be a long waiting time. We would like to encourage our patients to use the open surgeries for something that cannot wait for a booked appointment and that you feel you need to be seen that day.
Booked Appointments There are booked appointments available with a GP every afternoon we are open. These appointments can be booked up to 2weeks in advance. If you are booking for a routine appointment or something that is not urgent then we would ask you to try and book one of these appointments if appropriate.
Patient Network This is a voluntary group who support and advise on matters relating to service provision at the practice. We are fortunate to have a large and constructive group. If you would like to add your voice to this group please contact us. We would like to hear all types of feed back from our patients so if you would like to take part in the surveys we send out please inform us of your e-mail address.
Callow Compassionate Communities (Co-Co) Who are we? Callow Compassionate Communities is a voluntary organisation that provides support for local people at home, helping them to maintain their independence. We provide befriending for isolated or unsupported people which helps them to maintain links with their friends and activities in their community.
