  4. Orgs w Established HP Programs Nuclear Generation: (Enrico Fermi 2, ~1,000 employees) • Early Adopters (1997) in learning and implementing a robust HP program • HP tools and concepts are applied daily and are fully linked to all departmental and site processes • All employees are fully trained on the concepts and expectations • External agencies, INPO, USA, etc conduct assessments of the plant’s performance in this area • There is a site manager with responsibilities for Operating Experience (OE), HP, and Corrective Action processes. Staff of 6 for OE & HP work 4

  5. Orgs w Established HP Programs FOSSIL GENERATION: (7 fossil units + Peakers, ~ 1,500 employees) • 2003 – All employees are trained in HP Fundamentals • 2005 – All employees receive HP refresher course • 2008-09 – A series of significant unwanted events occur including a string of similar events related to high voltage switching. • 2009 -FG establishes a HP Champion to focus on Human Performance at each plant. Each site has HP lead/team. Fleet HP Organization has ~ 30 volunteers. 5

  6. Fossil Generation Continued • 2010 – metric employed quantifying # of HP events, Established 4 triggers to prompt full TWIN Analysis (Environmental Exceedences, Unit Derate, OSHA Recordable Injury, Cost to the budget from a single event in excess of $50,000 • Targets and performance to date: – <50 HP Resets…Actual: 68 – <175 HP Events… Actual: 168 – > 30 Fleet consecutive Reset Free Days…Actual 15 • 2011-Goals: Review and realign metrics to stronger safety focus and improved equipment reliability, develop a sustainability plan, Improve web site and information sharing, add volunteers on each site, improve employee HP knowledge, and more. 6

  8. Understanding our current culture Summary of replies from Committee members: Why do people not follow known safety procedures? • Time and Comfort • Easier - following procedures can take longer and result in a shorter break time. • Because that’s the way we’ve always done it, as long as I’ve been here. • Peer- pressure, don’t want to look foolish in front of others. • Think they have a “better” way than the procedure • The supervisor really wants this done in a hurry • Perceived time restraints • Complacency • Laziness, especially when nobody is watching • Not focusing on the job, distracted by personal problems, co-workers, etc. • I know my job, I find it demeaning that a procedure tells me how to do my job • Feel that procedure is wrong, or that they don’t “own “ it 8

  9. DIST OPS HP PROGRAM • Late 2009 – Organization experiences a fatality • Early 2010 – D.O. launches HP program and creates HP Manager role • 2010 primary focus - learning to use specific HP tools aimed at reducing the likelihood of field employees getting hurt 1 st qtr 2010 – HP Tool focus – Pre Job Briefs 9

  10. DIST OPS HP PROGRAM cont. 2 nd qtr 2010 – HP Tool focus – Observations – Hazard Recognition – Introduction to Error Recognition/Reduction Training 3 rd qtr 2010 – HP Tool focus - Practice SWO and Hazard Recognition skills 4 th qtr 2010 – HP Tool Focus – TWIN analysis during safety investigations 10

  11. 12. Human Performance Community of Practice (COP) • Human Performance COP was formed in December 2009. • Designed for 5 quarterly 4 hour joint sessions (3 have been delivered to date) • Approximately 75 individuals representing all areas implementing Human Performance • COP developed from existing Level 3 (Business Unit) Joint Safety Committees • COP has elements of training, practice, tool and concept introduction and sharing across the enterprise effort 12
