Agenda Item 5 • Formal Alliance of Partners committed to working together across Sutton, with people at the heart of the care we deliver, so we keep the population of Sutton both healthy and independent Page 2 • We started working together in April 2018 delivering the at Home Service and have committed to widening this to all community health services (adults and children) from April 2019
What do we want to achieve? Improved co-ordination between providers Improved flow Enhanced clinical input Page 3 Maintain people’s Increased citizen independence engagement Maximised wellbeing through additional investment in Agenda Item 5 prevention and proactive care
Sutton Health and Care will be designed and delivered across Agenda Item 5 three phases Preventative Proactive Supporting people to Reactive live healthy and Working together in Page 4 fulfilling lives in their multi-disciplinary At Home Service – local community teams and localities working together to to coordinate the look after people at care for people with home at times of ongoing health and changed need care needs 4
Why do partners think Sutton Health and Care should provide community services together? • We are the local partners and believe in Sutton • We can build upon our agreed ways of working – starting with at Home • It means we can focus in the next 2 years on what is the right Page 5 model of care for local people not on our individual organisations or competition • It means all staff can be involved in the work with the confidence of knowing what is agreed will be implemented in practice …....we can focus on what is important! What is our timeline? April 2018 July 2018 At Home Launch CCG announces interim award • SHC Partnership Board • LBS decides children’s services should be provided by SHC, staff employed by London Borough of Sutton November 2018 Contract for community services to SHC signed • Page 7 Start planning for the CCG community services inclusion into SHC • TUPE process commences • Apr 2019 Community services • Agenda Item 5 (adults and January 2019 children) provided Shadow arrangements commence • as part of Sutton Transition planning starts • Health and Care 7
