  5. Part 1 Invite Patient • Invite FPR or other patient in-person to become an ad-hoc (non-voting) member of QAPI or Gov. Board Committees • Share purpose of QAPI or Governing Board Committees • Share reasons why the patient is being invited • Share patient’s role during Committees and after • Ask if patient has any questions or thoughts regarding the invitation • Accommodate patient’s schedule to attend meeting • Include meeting date, time, and location • Ask patient to consider and reply by X date • If patient says yes, provide patient with My Notes & educate patient on topics covered & ask if they have any questions 5

Part 2 - Meeting • Prior to Meeting – Confidentiality Go through ALL reports that will be reviewed while the patient is present & remove any patient identifying information. – Each discipline evaluates/prepares report with directed questions for the patient. Use open ended questions. 1. What would be the best way to (fill in the blank)? 2. Based on this report, what did you hear that was important and how do you suggest that we assist patients with that? 3. What are challenges from your perspective to starting a new (fill in the blank)? 4. What are challenges from your perspective to changing (fill in the blank)? 5. What do you think is the best way to approach (fill in the blank)? 7

Part 2 - Meeting • At Meeting – Introduce all members attending by name & role – Ask patient to introduce themselves or introduce patient – Confidentiality  Observe all HIPPA rules and regulations  Do not refer to other patients by name  Do not discuss any case or patient specific details – Agenda : first 30 minutes should contain topics of interest to the patient & items Committee would like patient feedback or insight into – Use patient-friendly language when reviewing reports & data – Graph or charts : orient all who are present about what the graph or chart means or is showing (so the patient will not feel focused on) 8

  9. Part 2 - Meeting • Provide patient with My Notes so that patient can take notes during the meeting on items of interest • My Notes will assist patient with sharing highlights from meeting with patients after the meeting • My Notes provides basic information on QIP for patient 9

ESRD Quality Incentive Program (QIP) and Quality Assessment & Performance Improvement (QAPI) Centers for Medicare and Medicaid Services (CMS) developed the End Stage Renal Disease (ESRD) QIP to be the nation’s first pay for performance quality incentive program. That means that dialysis providers have an opportunity to enhance the overall quality of care that dialysis patients receive by reducing dialysis center payments if lower quality patient care is provided. Please use this sheet to write down areas of interest which were discussed and reviewed at the QAPI meeting and use it later as a reminder when sharing the information with other patients. 2014 Quality Incentive Program (QIP) Topics: • Hemoglobin greater than 12 g/dL(Blood) • Kt/V Dialysis Adequacy • Fistulas • Catheters • Patient Experience of Care Survey (ICH CAHPS) • Blood Stream Infections • Mineral metabolism (serum phosphorus and calcium) • Anemia Management Suggestions for improvement of facility performance 10

  12. Part 3 – After the Meeting • Facility – Include patient advisor’s attendance and participation in QAPI or Gov. Board Committee meeting minutes – Follow-up with patient directly on any hot-topic issues prior to the next QAPI or Gov. Board Committee mtg. • Patient – Two-way communication with other patients 1. Updates patients on topics discussed 2. If patient responses or feedback is requested, the patient advisor gets the feedback from other patients & shares it at the next QAPI or Governing Board Committee meeting 12
