Scope of Continuing Care Capacity Planning • Core Continuing Care Sectors – Long-Term Care (LTC) – Supportive Living Level 4D (SL4D) – Supportive Living Level 4 (SL4) – Supportive Living Level 3 (SL3) – Long-Term Homecare (LTHC) • Stand-Alone Sectors (not currently included in this planning process) – Hospice – Short-Term Homecare (STHC) – Adult Day Program (ADP) 4

Provincial Demand Forecast Scenarios • Forecast Drivers – Population growth – Population age mix – Disease leading to CC usage (expert panel) – Waitlist reduction (expert panel) – Utilization rate per adjusted capita (expert panel) – Desired mix of health services (expert panel) • Based on forecast drivers, forecast scenarios are created 5

  8. Capacity Planning Step 2. Executive Confirms Scenario • Forecast scenarios informed by forecast drivers are presented to the Steering Committee • The Steering Committee reviews and approves the forecast scenario • Forecasts are created based on the approved forecast scenario 8

Capacity Planning Step 3 – Zone Validation • The demand forecast forms the basis of Zone capacity planning, however the math is only one part of the process – Demand forecasts at the sub-zone service area level need to be validated with front-line knowledge and experience • Capacity planning flows from validated demand forecasts at the sub-zone service area level • The final product of the validation process is the Continuing Care Capacity Needs Assessment binder 9

  10. Capacity Planning Step 4. Provincial Steering Committee Review • The CCCNA binder is tabled again at the Provincial Steering Committee • The Steering Committee reviews and approves the final binder 10

Implementation • Based on these projections: – Capital priorities are determined – Priorities include potential Infrastructure projects, grant funded projects, Lodge Program projects, and other capital funding sources • Capacity changes are tracked on an ongoing basis on the Continuing Care Capacity Net New Dashboard • Capacity changes throughout the year feed into the next round of CCCNA planning 11

Continuing Care Placement Provincially: • Alberta released the “Access to a Designated Living Option in Continuing Care” policy in 2015 – Client choice and person centredness are key priorities in the policy – Transparent and consistent process – Clients preferred choice can be anywhere in Alberta – Approval dates in one zone apply to other zones – Uses the RAI HC as the standard assessment tool in all zones – Increased movement of clients to their preferred choice 12

Continuing Care Placement - continued In the North Zone: • North Zone created a centralized placement team in 2014 – Previously there were 3 separate teams, 3 separate policies, 3 separate processes – Increased access to services for clients – Strong advocacy for the zone on provincial committees – Consistent processes across the zone in delivering services – Creation of partnerships with other Zones – Emergency response improvement – Ability to learn from each other, and create program improvements – Ability to ensure clients are treated equally regardless of geography 13

North Zone Continuing Care Access 1-855-371-4122 • North Zone staff provide information, navigation, access and intake into continuing care • 7 day per week staffed with RN’s • Inquiries for information or entry to the health system for home care, supportive living or long term care • Consistent assessment tool used when working with getting the client access to the services they need 14

Palliative Care in the North • Program planning to develop a palliative care program to support clients across the zone • Key goals: – Allow clients the opportunities to make choices – Support service empowering front line care – Front line education will be a key feature • Model in development 15
