Utility bill (water, garbage, or electricity bill) iii. Voter registration iv. Pay stub v. Correspondence from a government agency d. Student’s immunization record (most recent with student’s name and date of birth on it) i. Please complete the Health History Exam Form e. Student's birth certificate, passport, or baptism certificate f. Complete and print the Home Language Survey (FUSD Home Language Survey). g. Complete and print the Supplemental Student Information (Supplemental Student Information). 2. For your records, please download a copy of our Parent/Student Handbook. 3. In order to complete your registration please schedule an appointment, and bring all necessary documents for final verification. 5 You will then click on the “ Appointment Calendar ” link to schedule that appointment.

School Placement During your enrollment appointment your child will not be placed at a school; placement comes once we’ve established availability at each school site for each grade level. ● In the event that there is not space at your “home school” your student will be “overloaded” to a school with space. ○ When overloading, we try to keep students as close to their home school as possible and within the attendance area. ● Placement is done by enrollment date and time (with the exception of kinder priority students, which are by Lottery number). ● If overloaded, your student will be placed on a waitlist at the home school ○ Students are “called back” to the home school, in order of waitlist number, as the space becomes available. 6

Enrollment Tips ● Enrollments are finalized when all documentation is received by Student Support Services - Pre-registration summary, proof of birth, updated immunizations, proofs of residency, homeowner/renter statement of residency ● Existing residency in Fremont is required for enrollment. A family must currently reside in Fremont - being in escrow does not establish residency. ● The name of the parent/guardian who comes to the enrollment appointment needs to appear on the proof of residency documents. ● During TK-K Priority Enrollment, there is no priority based on appointment date time. All students enrolled during the priority window (Jan. 14-Feb. 22) are eligible for kinder lottery, if needed. 8
