Aim of the day We will hold conversations about integrated care systems and share examples of good practice about where collaboration has helped to build solid relationships and trust across social care and health
Integrated care The person’s perspective is at the heart of discussion about integrated care. Achieving integrated care requires those involved in planning and providing services to ‘impose the person perspective as the organizing principle of service delivery’ (Shaw et al 2011 after Lloyd and Wait 2005)
Creating a workforce for integration Involves whole system collaboration To achieve genuine integration of services based around the person, we need to acknowledge and overcome resistance to change and transition; as acknowledged in Beyond Barriers (CQC 2018): “Joined - up care… requires a workforce equipped to move between health and social care. Workforce planning needs to create the skills and career paths that allow people to work flexibly across the system as services evolve over time to meet the population’s changing needs.”

Methodology • A goal was set it was NOT a target! • Bottom up approach • Staff were empowered to make decisions • No fear of failure • Testing new approaches - flexibility • Wasn’t a structured project management approach
Snapshot of the activity: Teams quickly stepped into action, testing a whole range of ideas in people’s homes, in community settings, in primary care and in hospital settings to support people at different points in their journey. This included: Initiating multi-disciplinary meetings across hospital and community settings focussed on proactive Establishing, building person-centred care. Over 40 and growing links with 288 voluntary and held over the community activity challenge 100+ Personalised care and support plans completed New connections and referrals Establishing COPD peer support group 8 Developed and tested integrated team running extended hours New, integrated and multi- services in a hospital to prevent disciplinary approach to unnecessary use of services, GP weekly sessions discharge through the creation of and hospital primarily. a Home, Safe, 40 sessions run, Sooner campaign
Stockton’s 100 day challenge: the context Early results were in.. Early Results from the challenge were promising… • Developing a system-wide consistent approach to personalised care and support planning • Almost 300 care plans completed over 100 days • Over 100 new referrals and connections to community Learning Agenda themes for teams groups and services • Over 40 new multi-disciplinary team meetings held • 40 out-of-hours sessions run focussed on admission avoidance • 41% reduction in A&E attendance, and 19% reduction in 1 day admissions from an urban-based team • 35% reduction in DTOC across the hospital (combined with other efforts)
