Plan has a $400 annual deductible. (Deductible is the amount you pay before the insurance begins to pay their portion/percentage). Once the deductible has been met, the insurance company will then cover 80% of the medical bill, and the student will pay 20% Student = First Insurance Company = $400 of the 80% of charges after year, then 20% the first $400. of charges after that.
Insurance Company will mail card to your address. Present Card to Health Provider at your visit so they can bill the insurance company. Remember, UIS Health Services is different, and they do not bill back to any insurance company. You will need to obtain a “Walk - Out statement” and file the claim yourself.
