The Primary School The Primary School currently welcomes approximately 512 students each day in grades K-3. The children are placed in 26 classrooms with a variety of special area, resource & support classes.

Required Forms • Completed Health History Form. • SCHOOL PHYSICALS: When a child enters school for the first time, New York State Education Law requires a health examination from a Physician. You can plan for this by taking your child to your private physician or clinic and provide us with a copy of said physical. If you are unable to have your private physician complete the physical please contact your nurse, who will schedule a physical with the school doctor. If you fail to provide a physical by October your child will be scheduled to have a school physical. • DENTAL CERTIFICATES: A law was recently enacted that expands health screenings to include the dental health of students in New York State. After September 1, 2008, when we require that your child have a physical examination, we will be requesting a dental certificate as well. Attached is a certificate available for you to take to your child’s dentist and once it is completed, it should be returned to the School Nurse as it will be filed in your child’s Cumulative Health Record.
Let us KNOW!! One of the best tools in our tool box is KNOWLEDGE!!! • Medical Conditions • Medications (even if they are only taken at home) • Allergens: Life threatening or mild, we need to know. • Food Related illness: Lactose intolerance or Gluten intolerance.
• Medications in school: Any medication your child should need in school must be delivered to the health office by a parent/ guardian and must be accompanied by a physician’s order. This applies to all medications including over the counter medications. Please see Mrs. Kubow if you need further information.

RegistrationProcess Registration packets have been mailed to your home. If you have not received one, please pick one up at the table. If you received your packet, and have all necessary documents please see Kelly in the lobby Faith is available for any medical questions/concerns
Your Registration is complete when: • Original birth certificate • Proof of Residency • Completed registration forms • Record of immunizations • Copy of most recent physical *Screening appointment will be provided*
Open Registration • If you do not have all necessary documents this evening… . • Open Registration Week: February 3 th – 7 th from 9 am to 1 pm. (Translators will be available on Feb.4 th and 6 th )
Screening New York State requires that all entering kindergartners be screened.
• Members of the teaching faculty will screen the children using the “Speed Dial” (Developmental Indicators for the Assessment of Learning) • DIAL screens fine motor skills, gross motor skills, concept development & communication/language skills Screening will take place April 2 nd , May 7 nd and 14 th in the • evenings starting at 3:00 pm • Children who are students in the Questar Pre-K program will be screened during the school day on April 2nd
