Whole Person Care Program • Pilot Program in Ventura County • Target Population: Gold Coast Health Plan Multiple Chronic Conditions 97.3% (MediCal) • HUMS: High Utilizers of Medical Services: Mental Health • Disorder =/> 4 ER visits &/or =/> 2 more Hospitalizations Substance Use 40.5% • High Risk Homeless Population: Disorder 24.4% • Intensive, multi-disciplinary care coordination • Community Service/Health Workers Homeless • RN Case Management 27.7% • Licensed Behavioral Health Clinicians • Drug and Alcohol Counselor 2

Delivery System • Intensive, multi-disciplinary care coordination • Engagement using CHWs • Build relationships & Foster self-management skills • Address social/behavioral barriers to wellness • Wraparound supports – food, transport, shelter/housing leveraged through community partnerships • Expansion application • HCA Outreach CARE PODS • Recuperative care – 12 beds operated by National Healthcare Foundation at Salvation Army transitional living center facility 3

Services we provide: Intensive, multi- (Medical, mental health, disciplinary alcohol and drug, social care services) coordination Frequent (Help navigating the check-ins system, addressing from barriers, building capacity community for self-management) health workers Field-based (RN, BH Clinician, Alcohol services (at home or in and Drug Treatment the Specialist) community) 4

HCA Mobile Outreach Care Pods • Self-contained portable units with showers & exam room • Mobile care unit strategically located in areas where the homeless population congregates that allows access to medical and social services. • Staffed with multi-disciplinary personnel, community partners • One Stop model • Focus is: • Basic hygiene • Limited-scope medical services (screening, vaccinations, TB testing) • Field treatment of some conditions (lice/scabies, basic wound care) 5

Care Pod Events • 3-4 hour events • Staffed with: Nurse • Behavioral Health Clinician/Alcohol and Drug Treatment Specialist • • Community Health Workers or Community Social Service Workers • One Stop Partners/Volunteers • Cell phones Benefits (MediCal, CalFresh, General Relief) • Mental health/alcohol & drug referrals • • Housing/shelter referrals (HMIS) • Employment • Under Development Animal services, haircuts, clothing, services for veterans, bag lunch/hot meal • 7

  8. Care Pod Needs • Volunteers! • Open through Close (Mondays 11:30 to 4:30) • Start-up and shut-down procedures • Monitor showers • Supplies Water bottles, shelf-stable snacks • Hygiene supplies • Clothes -- esp. socks, undergarments, jackets, shoes • • Blankets, sleeping bags • Sack lunches 8
