Health Center Program - Overview For 50 years, HRSA-supported Health Centers have provided comprehensive, culturally competent, quality primary health care services to medically underserved communities and vulnerable populations. Health Centers are community-based and patient-directed organizations that serve patient populations with limited access to health care.

FY2013 • Midtown Community Health Center - South Salt Lake - to serve 4,300 patients by 2015 HIGHLIGHTS • Paiute Indian Tribe of Utah - Richfield and Kanosh - to serve 1,234 patients by 2015 • Mountainlands Family Health Center - Vernal - to serve 3,308 patients by 2015 Health Center Outreach and Enrollment Supplemental: $150M awarded to 1,159 Health Centers to expand current outreach and enrollment activities and facilitate enrollment of eligible patients and service area residents in to affordable health insurance coverage through the Health Insurance Marketplace, Medicaid or CHIP. • Utah $1,374,648* o Cumulative Trained: 89 people trained o Cumulative Assists: 29,186 people assisted o Cumulative Apps Submitted: 4,421 applications o Cumulative Enrollments: 3,263 people enrolled *October 2013 – June 2014

ALLOCATIONS • Utah Partners for Health – Midvale - to serve 5,666 patients by 2016 FY2014 • Community Health Centers, Inc. - Glendale - to serve 3,910 patients by 2016 HIGHLIGHTS • Wayne Community Health Centers - Escalante - to serve 2,085 patients by 2016 Health Center Mental Health Service Expansion – Behavioral Health Integration: $55M awarded to 221 Health Centers to establish or expand behavioral health services to over 450,000 patients nationwide. 4 th Street Clinic - $250,000 • Health Center Expanded Services Supplemental Funding (September): $300M in formula based supplements awarded to 1,195 Health Centers to help expand service hours, hire more medical providers and add oral health, behavioral health, pharmacy and vision services. • Utah 12/13 Health Centers $2,762,734 • 41 jobs • 11,514 new patients

  17. HEALTH CENTER TRUST FUND ALLOCATIONS Health Center New Access Points: $100M awarded competitively nationwide May 2015. Grantees FY2015 will be operational by September of 2015. HIGHLIGHTS • Midtown Community Health Centers : Ogden – Homeless • Mountainlands Family Health Centers : Provo - Homeless • Family Healthcare : Cedar City (Co-located with SW Behavioral Health) Health Center Expanded Services Supplemental Funding (September): $350M in formula based supplements. Applicants must propose to use at least 80 percent of the Expanded Services funding to increase capacity to provide one or more of the following eligible services: medical services; oral health services; behavioral health services; pharmacy services; and/or vision services. Up to 20% of funding may used to increase the availability of new and/or existing enabling services (e.g. care management/coordination, health education, translation, transportation, etc.) • Utah $2.8M (estimated amount) • 10,000 Patients – (new & existing estimate)

Between 2012 to 2014 Utah Health Centers have expanded access to: • 9,052 uninsured patients; • 6,957 privately insured patients; • 2,248 Medicaid/CHIP patients; • 2,077 Medicare patients. *Actual Data SOURCE: HRSA 2013 & 2014 Unified Data System **Other professional health visit can be visits with a broad range of health providers other than those listed under Table 5. Examples include: occupational therapists, Physical therapists, dieticians, nutritionist, podiatrist, speech therapist, acupuncturist, dental therapist, Naturopaths, acupuncturists, community health aides/practitioners, and chiropractors, etc.
