W hy is collect ive leadership needed? Poorly im plem ented hierarchical leadership can contribute tow ards patient safety failures, blam e cultures, under-reporting of errors etc. The challenge • Pow er im balances m ay occur betw een junior and senior m em bers of staff. • Non-standard practices becom e accepted. • Scapegoating, clim ate of fear of failure rather than a focus on learning and im proving from failure.

Co-designed by health professionals, quality and safety experts, patient advocates, and researchers. Using t he t oolkit Co-Lead toolkit m odules take the form of 1 hr long w orkshop sessions Sessions m ay be integrated into norm al 1 m odule every 2 team m eeting opportunities such as journal w eeks/ m onthly club etc. Set of recom m ended m odules to ground all Team s undertake m em bers in key concepts of collective 6 core m odules leadership. Team s select Specifically address key areas of collective targeted m odules leadership chosen by the team .
W hat is needed? Each dow nloadable m odule package contains step-by-step instructions and resources to facilitate a w orkshop session. 1 -2 team m em bers facilitate the session, w ith different m em bers facilitating each tim e. The rest of the team !
