For over 50 years, HRSA-supported Health Centers have provided comprehensive, high- quality primary and preventive healthcare services to all individuals, with or without insurance, regardless of ability to pay. Health Centers are community-based, patient-directed organizations that eliminate geographical and financial barriers and serve populations with limited access to healthcare.
HIPS helps qualified individuals and families pay for their monthly health insurance premium. After Marketplace subsidies, participants receive additional credit to reduce or completely eliminate their monthly premium payment. The program is funded by the Intermountain Community Care Foundation and administered by AUCH.
Utah SmartCare helps improve health and well-being while reducing costs for treating patients with mental illness through the use of cutting-edge smartphone technology. The app helps patients stay connected to their doctor between appointments.
Utah Healthcare Corps Members are placed at Community Health Centers and other health-based organizations and work to remove cultural, linguistic, financial, and geographic barriers to quality and affordable healthcare for medically vulnerable populations. The program is funded by the Corporation for National and Community Service, UServeUtah, and administered by AUCH.
The Introduction to Care Coordination training consists of an intensive 2-day face-to-face session, online modules, and forum calls. This training is an excellent workforce development opportunity which benefits medical assistants, community health workers, case managers, and others with clinical support roles.
Community Health Workers (CHWs) come from the communities they serve. They understand the needs of their community and act as caring neighbors to help patients overcome social and medical barriers that affect their health. The project is funded by Intermountain Healthcare and administered by AUCH in partnership with the Utah Public Health Association.
