  8. Team members learned new things to apply in their community. • Ken • “I learned some great program results from speaking with other teams at the Atlanta meeting.” • “We have adopted many of the tools that Larry Cohen first demonstrated in his Prevention Institute presentation.” Team Osceola • Belinda • “This program has enhanced my knowledge about community leadership and partnership development” • Dorie • “Continued collaboration and open dialogue is a key component to the success of any initiative and this program helped reinforced these concepts.” 8

  14. Planning to enable a vision of a future state really helped. Past strategic planning has taken • into account current health issues, but has only addressed immediate steps over the next 12 months to advance improvements. Team Osceola Creating a vision of what the ideal • state would be (a healthier chronic care population) opened us to different ideas on how to approach the problem. 14

  15. While not fully started, some project outcomes have been achieved. EMS is planning with us. Team Osceola A patient navigator position has been approved and a candidate search is underway. The target population has been defined. 15

  16. NLAPH guidance really smoothed the way. • Ken • “Milano helped us narrow the scope for the project to something more achievable.” • Belinda Team Osceola • “Milano has been a very hands on coach, and has challenged our team to think out of the box while incorporating concepts and tools gained through our program participation.” • Dorie • “Team discussions in Atlanta, dissecting Myers Briggs results among our team members, webinar discussions and Milano’s attention to our needs all played a major role in bringing our Osceola County program into clear focus.” 16

My organization will be working with them.” • Belinda Team Osceola • “As team lead for this project, I have greatly appreciated and embraced the concept of team diversity.
