  4. 2020 Projects • Partner on the Care Mobility ty Rewards Program. Assist in the limited launch of the program to reduce hospital readmissions for Medicare recipients in South King County. • Increase coordination of South th King County transportation providers. Organize a Roundtable Discussion for South King County transportation providers to increase coordination of South King County transportation providers and establish regular communication channels and resource sharing. • Increase transportati tion educati tion to h harder to r reach populati tions in South King County ty. Increase transportation education and organize travel trainings for harder to reach populations, including isolated community members and multi-family housing residents. Lead awareness campaigns of transportation services that are culturally competent and utilize new methods such as transcreation and pictograms for limited English proficiency populations.

Care Mobility Rewards Program • An incentivized transportation program that provides rides for post-discharge medical appointments to reduce re- admission rates. • Patient eligibility criteria: ▫ Medicare and dual-eligible ▫ Older adults in SKC ▫ Have a higher risk of re-hospitalization ▫ Do not have secure transportation for their post- hospitalization appointments

Care Mobility Rewards Program • CTAA Grant received in August 2019 • Partnership with Valley Medical; planning conducted in fall/winter 2019. • 3-month pilot program runs from February 18 th through April 15 th 2020 (for enrollment). • Grant ends on May 15 th , 2020. ▫ Evaluate and secure sustainable funding in summer/fall 2020.

  10. Care Mobility Rewards Program - Steps 1. The discharge planner identifies the patient during discharge. 2. They have a conversation about the program and the transportation resources. 3. The discharge planner fills out the patient enrollment form and fax it to Hopelink Brokerage. 4. Upon enrollment, the patient receives 2 credits (equals to 2 free round-trip rides). 5. The patient calls Hopelink Brokerage to schedule a ride for qualifying medical appointments and/or items listed on their action plan. 6. Once the patient completes the ride, additional credits are awarded.

Collateral Development • In partnership with KCHA and SHAG, held focus groups at 2 low-income housing facilities in South King County • Reached out to 15 residents in total • Heard to reform the collateral for fewer words & simplified language, bigger font, better graphics, clearer language, and to make it a brochure

  13. Medical Staff Training • Held a test training on January 28 th with 4 participants • Heard to add wellness-related trips (grocery shopping, senior center lunch, etc.) to Action Items • Held final pilot training for discharge planners on February 13 th with 17 staff members

  14. Medical Leadership Buy-in • Partnering with Valley Medical to test Medical Leadership pitch • The goal is to gather constructive feedback and be able to use the pitch at other medical facilities for future funding • Sustainability

Up-to-Date Timeline • Pilot launched on February 18 th , 2020 • Actively identifying patients, sharing about the program, and receiving calls • Collecting data ▫ 6 total individual trips requested as of ¾; 2 round-trip and 2 cancelled • Utilizing faxed forms, may be following up with registered patients to encourage them to use rewards before they expire • Grant ends on May 15 th , 2020
