  4. Internal Audit Findings • Poor Compliance with completion of documentation • Equipment section rarely completed • Large document – difficult to use • Duplication of information • Lack of clarity around who should complete which sections and when • Trainee doesn’t have ‘ownership’ of documents

UKAS Findings • 5.1.6 Competence assessment Following appropriate training, the laboratory shall assess the competence of each person to perform assigned managerial or technical tasks according to established criteria. Reassessment shall take place at regular intervals. Retraining shall occur when necessary. • Lack of evidence of competence • Lack of evidence of consistency in training and assessment • Unable to gather evidence of technical procedures from Shire

Methods of Competence Assessment • Direct observation of routine work processes and procedures, including all applicable safety practices • Direct observation of equipment maintenance and function checks • Monitoring the recording and reporting of examination results • Review of work records • Assessment of problem solving skills • Examination of specially provided samples

Aims of the new documentation • Complement Departmental training policy • Engage trainees and GT trainers in the competence recording process • Ensure consistency in training delivery and assessment of competence • Ensure consistency in determining the success of a procedure • Formalise process for gathering evidence of competence

Process • Delivers training in line with training plan and objectives • Outcome of each process assessed and signed for Trainer • Performs DOP assessment of trainee performing procedure • Adds comments to Competence Review Form Training supervisor • Document all work undertaken on the Competence Document • Completes all sections of Competence Document Trainee • Review completed Competence Document • Adds final comments to Competence Review Form Training Supervisor
