Introduction • How we maintain service users nutrition and hydration across the Trust.
Catering Services Ensuring that all service users have: •Enough to eat •A choice of foods to suit all diets and appetites – Dieticians – Range of menus – In right sort of packaging/containers •Help with eating if needed – Individualised care planning – Carer involvement – Protected meal times – Housekeepers – Volunteers

Focus on Physical Health Care •Senior Multi-professional Group to promote good physical health care •Nutritional care group •‘Energising for Excellence’ events •Training workbooks developed Nutrition & Hydration Diabetes Vital Signs •Physical healthcare training days
Monitoring •Modern Matrons observation visits including weekly weight monitoring reports •Nutrition group monitors all clinical incidents relating to food and nutrition and follows each incident through to ensure suitable outcomes •Nutrition & Hydration audit tool •Food wastage audits •Service user satisfaction monitored through PALS •Essential Standards visit programme involving Governor Members
Monitoring Benchmarking tools including: • Ombudsman’s Care & Compassion Report February 2011 • Privacy & Dignity reports - Care Quality Commission 2011 • Essence of Care • Chief Nurse – High Impact Actions
Future Plans • Revised Guidelines for Nutrition & Hydration including new fluid balance monitoring • Nutrition & Hydration audit tool reviewed and piloted in community and inpatient settings
