What Happened? • Driver was loaded and inbound to Frac • Driver went off the edge of the road • No injuries • What was spilled? – Fuel – Engine oil
Contributing Factors to Rollover • Poor visibility – Dark country road • Weather – Pouring rain • Road conditions – Soft, saturated shoulder; wet, shiny road surface, narrow lanes with no center or edge lines • Oncoming water trucks – Headlight glare, did not give right away • Poor communication – Water/sand trucks were closer than communications suggested
Root Cause/Learnings • Driver Error • What driver should have done to prevent this rollover PAUSE AND STOP THE JOB – Driver was aware of: 1. Oncoming Traffic 2. Narrow Roads 3. Glare from Headlights 4. Saturated Shoulders
Action Plan • Shared Incident on weekly HSSE call • Stressed Right-of-Way procedures to all carriers 1. Loaded Trucks, Tractor Trailers, Straight Trucks 2. Use common sense when yielding • Safety Meetings with drivers 1. Pause and Stop the job 2. Strongly discussed right-of-way procedures
