November 18, 2019 Dear Ms. Cougar: On the basis of recommendations and the documentation provided, your request to receive the services available at the College of Charleston for students with disabilities is approved. To complete the final steps of this process and receive your Professor Notification Letter (PNL), you have two options: 1. You are welcome and encouraged to meet with me to review your diagnostic documentation together, discuss the academic accommodations and services you are eligible to receive, and talk about other resources on campus that may be beneficial to you ……. OR 2. Complete the CDS/SNAP Acknowledgement Form independently and submit it in one of three ways (listed below). The accommodations you requested on your application will be reviewed. If I have a question regarding the accommodations you requested on the application, I will contact you.
For students who want to pursue option 1 (per approval letter): If you want to schedule a meeting while you are in Dublin, London, or Rome, we can conduct a video conference with you. The best times to conduct these meetings would be between 9am-2pm EST (United States) This would mean anytime between 3pm-8pm for you.
November 29, 2019 Re: Imma Cougar SID#: 12345678 Dear Professor: This letter is to confirm that Imma Cougar, a student in your class, has provided the College of Charleston with proof of a disability. Accommodations are required under Section 504 of the Rehabilitation Act of 1973 and the Americans with Disabilities Act of 1990. These accommodations are to provide equal access to the course and help measure the student’s abilities rather than disabilities and are not designed to provide an unfair advantage or change course requirements. The student may be discussing the following accommodations with you. Documentation supports the following accommodation(s): Extended time (time and a half) on tests, quizzes, and in-class assignments in a room less distracting than the classroom The Center for Disability Services (953-1431) can facilitate this if notified in advance. The Professor is responsible for arranging delivery and retrieval of tests. Permission to audio-record lectures
