Filing Requirements • All lodges, chapters, councils, commanderies, and collateral bodies with less than $50,000 gross receipts • Annual filings are required by the 15 th day of the fifth month (next business day if the day falls on a weekend or or legal holiday) after the close of your tax year • Must register at IRS.gov before you can file your 990-N

  3. Required Information to File • Your organization’s EIN, a.k.a. TIN • The tax year for the report • Legal name and mailing address • Any other names the organization uses • Name and address of a principal officer (president, vice president, secretary or treasurer) • Web site address if the organization has one • Confirmation that the organization’s annual gross receipts are $50,000 or less • If applicable, a statement that the organization has terminated or is terminating

IRS Non-Profit Status Restoration • Non-Profit status is lost if an organization fails to file a 990-N, 990EZ, or 990 for three consecutive years • Need IRS Form 1024 current revision January 2018, “Application for Recognition of Exemption under Section 501 (a)” (fillable pdf available on IRS website) • Need IRS Form 1024 instructions, “Instructions for Form 1024” • Both are available at https://www.irs.gov/forms-pubs/about-form- 1024 • Need IRS Form 8718, “User Fee for Exempt Organization Determination Letter” Current revision March 2018)

State Application • To be completed after receipt of IRS Determination Letter • Submit the following: • Request for exemption from MD income tax • An explanation of the nature, purpose, and scope of your Organization • A copy of the IRS tax Determination Letter • A copy of your organization’s By -Laws and • A copy of the latest financial statement of your organization • Organizations soliciting charitable contributions in MD are generally required to register with the Office of the Secretary of State as a charitable organization • Go to https://sos.maryland.gov/charity/pages/non-profit-organization.aspx

What do I need to submit ? • Form 1024, 8718, and $600 check payable to United States Treasury • Completed IRS Form 1024, current revision January 2018, “ Application for Recognition of Exemption under Section 501 (a)”, fillable pdf available on IRS website • Need IRS form 1024 instructions, instructions for Form 1024” • Both are available at https://www.irs.gov/forms-pubs/about-form- 1024 • Need IRS form 8718, “User Fee for Exempt Organization Determination Letter”, current revision March 2018” • Conformed copy of the corresponding organizing documents namely, Charter and By-Laws

  13. • Statement of Revenue and expenses for current year (Within 60 days of application filing date) and 3 prior tax years • A check for $600 payable to “United States Treasury” • A certificate signed by the secretary of the parent organization, under the seal of the organization, certifying that the subordinate lodge is a duly constituted body operating under the jurisdiction of the parent body (For Section 501(c)(8) or 501(c)(10)). • Remember to read the checklist at the end of Form 1024 and read all instructions on the form and instructions carefully
