Howard Brown Health Mental Health Quality Improvement Presentation Service Plan/ Treatment Plan Revision • Project Rationale: Review clients served by the RW Part A for Mental Health services to assure compliance on the following: • Standard 3: Measurement & HRSA Service Plan Requirements Measure, H1 Initial Mental Health Service Plan • Standard 7 & 8: Measurement: Progress in Treatment and Outcome of Treatment, Item H8, Progress towards one goal. • The data analyze reflect the audit period: March 1st, 2016 to February 28th, 2017 • Baseline Data: 42 Individuals, 39 Not Englewood, 3 Englewood Individuals with a developed or updated treatment plan = 19 • (45%), Not Englewood = 18 (46%), Englewood = 1 (33%) • 10 Individuals were Phone Screened and Refer Out

Define the standards on how information should be documented. Found that some charts without treatment plan were • clients assigned to group therapy.  Take a look into this and decide how could be solved.  Sustained Improvement: Continue completing chart audits to assure that proper documentation is complete and to identify issues that need to be resolve.
