Growth of Home therapies • Clinical lead for home therapies from 2015 • Merging of PD and HHD nursing teams to create ‘Home therapies team’ with band 7 lead from Jan 2015 • 4.2 WTE band 6 for PD, CKD education, anaemia management and conservative care • 2.88WTE (2x band 6, 1x band 5, 1x band 4) for training and community support for HHD pts.

Developments in 2016 • Peer support programme • Drive for pts to use PatientView • Nx stage campervan tour • Home therapies fayre in addition to CKD information seminar • A drive to raise awareness of home therapies amongst patients AND staff • Medical insertions of PD catheters
Work in progress • Structured training document for HHD pts • Consistent training techniques by all staff • Increase in teaching skills/confidence in staff • Developing minimal care philosophy across all units
Promotion of home therapies • Early discussion about the concept and benefits of self care • Clear message for PD as first dialysis option, then HHD • Pt education during low clearance clinics, home visits and information seminars • Education by staff experienced in home therapies
Future plans • Collaborative working within nursing teams to ensure consistent service • Raising awareness of home therapies with patients and staff • Development of minimal care in all dialysis sites • Continue patient education after starting dialysis to encourage self care
