The Mission and Vision of Wagner Community Memorial Hospital Avera It is the objective and purpose of Wagner Community Memorial Hospital Avera to provide care and treatment of the sick, to provide care and support of the aged, disabled and indigent and to provide for those in need of hospitalization. The vision of Wagner Community Memorial Hospital Avera is to be the provider of choice in the area; to be financially stable and strong; have a solid physician base and physical plant; and to have good relations with other community healthcare providers.

• Concept initiated by CMH Physical Therapy Department • CMH provides Certified Athletic Trainers to local school districts • Grant funding to start concussion (ImPact) testing and protocols for all student athletes. • CMH receives direct referrals back into the system • Over 850 baseline Community Partnerships assessments Sports Medicine Outreach
• Astoria School District owned property adjacent to CMH for High School Football program • City of Astoria owned and needed to cap a defunct city landfill • CMH needed additional land to bring a new Cancer Center to community • The groups worked together to create a new Sports Complex on old landfill site – CMH provided majority of the funding. Baseball, Football, Soccer, and other community activities now available on artificial turfed Community Collaboration complex. • CMH obtained 7.5 acres of CMH Field adjacent property for expansion
Reduce Unnecessary ER Visits and Improve access to Primary Care • CMH opened a new combination Urgent Care and Primary Care facility in an off-campus location in 2013. • Added 5 net new primary care providers. • Utilized the Rural Health Clinic model. • CMH is opening a duplicate clinic in Astoria in Fall of 2018
CMH/OHSU COLLABORATION
• 2011 - Partnership created • Medical Oncology Clinic started part-time 2 days per week • 2012 – Increased to full-time and moved into 5,000 Sq Ft Clinic and Infusion Center • $300K Urban Renewal grant from City of Astoria • 2013 – Increased to 2 full-time Medical Oncologists • 2017 – Opened $16 mil dedicated Cancer Center including Radiation Therapy • CMH-OHSU Knight Cancer 2 full-time Medical Oncologist & 1 Full-Time Radiation Oncologist. Collaborative
