Cooperative Governance Assessment Assessment Expectations • Each Director questionnaire requires about 60 minutes to complete • Each cooperative member questionnaire requires around 25 minutes to complete • Assessment can be either: – Guided by you – Self-guided • Please provide participant with a paper copy of the appropriate questionnaire (Director or Cooperative Member), a writing instrument and a place to sit • Ask that the participant complete the questionnaire independently in front of you • Remain close by to respond to any questions that may arise • Collect the completed questionnaire
Cooperative Governance Assessment Assessment Expectations (continued) When completing the Questionnaire : • Record date, name of cooperative, and participant’s number on first page of questionnaire • Take care to ensure that each question is answered When addressing participants: • Be respectful and kind to all participants, be patient and understanding • Address the participant by his/her first name to make the individual comfortable, but do not record the name on the form • Never suggest an answer to a participant, focus on understanding the participant’s response
Cooperative Governance Assessment Assessment Expectations (continued) Be on Time, Maintain Focus • No absenteeism or late arrivals will be permitted • Dress appropriately in business casual for field work • No cell phones allowed, do not interrupt assessment process • Do not promise/provide any incentive to participant • Review assessment questions and be sure you understand the meaning. If you have questions, let us know. If There Is Confusion • First, ask the participant the statement as written – If the participant does not understand, repeat the statement slowly as written – If the participant still does not understand, ask what she/he doesn’t understand and explain the statement • If a participant will not provide an answer to a question or does not have an answer, note this on the questionnaire and move on to the next question
