Calhoun County Home Visiting Hub Funding History: • MIECHV Hub planning grant, small portion of 1 st year Hub grant, LLG funds - & individual home visiting program grant funds (supporting shared goals!) Hub Priorities: • Engage 211 as the center of referrals – allowing existing referral processes to also continue • Simplify the referral process – decision tree, closing the loop • Collaboration to promote program coordination, alignment and informed referrals – joint brochure and training
