Lake County Catholic Charities PSS Quality Improvement Presentation: Indicating progress towards goals in Psychosocial case notes On the last CDPH audit of psychosocial services, Catholic Charities received a low score on 2 standards. These are standards 5 and 6 which required progress documentation within individual and group notes. Standard 5 : Percentage of individual psychosocial service patients who have a service plan with progress notes documenting progress toward service plan goals after each session. Catholic Charities Score: 0/1 met the standard (0%) Standard 6 : Percentage of group psychosocial service patients whose progress towards goals is updated at minimum twice a year. Catholic Charities Score: 2/25 met the standard (25%) Goal : 80% of psychosocial charts reviewed will have progress notes that contain progress made towards a goal

It was determined that some staff did not know that progress for the goal being worked on needed to be documented in each case note. So then what? We assumed it would be easy to implement new procedure internally to ensure progress is being documented after each visit for psychosocial services. An email was sent out making sure staff were aware of this requirement. How was progress measured? To see if we were making improvements : The Program Director reviewed 20 charts for the month of September and October. Results: No significant change was noted. Action : The Program Director had clarifying discussions about what was needed with the lead Psychosocial staff. Lead Psychosocial staff agreed to communicate this to others and to spot check notes. Sample statements of progress and evidence of progress towards a goal sent out. Next Action: The Program Director reviewed 20 charts for the months of November and December.

The goal was 80%, so we still fell short, but it is a great improvement from zero! Conclusions : We made significant progress over the past two months but still have work to do. It is difficult to get people to make changes, especially when it adds time to a process, especially when it is administrative. To keep improving : The Program Director will provide results to lead staff and ask that they review notes and feedback with staff. Notes will continue to be spot checked with the expectation of including progress in each note.
