  3. Open Enrollment October 21, 2019 - November 8, 2019 During Open Enrollment, eligible active employees may: • Enroll in Benefit Coverage • Change Plan Option and/or Vendor • Enroll eligible dependents • Drop covered dependents • Decrease or increase coverage tier • Discontinue benefit plan option(s)

  4.   5. User IDs and Passwords • Check User ID and passwords of both SHBP and GaBreeze Websites. • Review and/or update email addresses to ensure a faster response in case of needing password resets. • Review, designate and/or update beneficiaries during Open Enrollment, especially for qualifying events.

  6. Confirmation Information • Employees will receive a confirmation page upon successfully completing their Open Enrollment elections. • PRINT AND KEEP A COPY OF CONFIRMATION. • Additional changes are permitted online during the Open Enrollment period. The last change made by November 8, 2019 will lock in the benefits election made for PY2020.

  7. What happens if no changes need to be made? • Employees who do not make an election will be defaulted/rolled over to the same plan option previously selected for 2019. • Employees not currently enrolled in SHBP/Flex in 2019 will be defaulted to no coverage in 2020. • Exception : Flexible Spending Accounts do not roll over.

  8. Employee’s Responsibilities • Read and make sure to understand the information in the SHBP Active Employee Decision Guide and Flexible Benefits You Decide Booklet. • Review the 2020 Rates and Plan Options. • Confirm that the SHBP Tobacco Surcharge question was answered appropriately. • Check your payroll deduction to verify that the correct deduction amount has been made. Health Insurance premiums are taken from the paycheck paid on the 15 th of every month. Flexible benefit premiums are taken from the last paycheck of the month. • Update any changes in contact information by going online to Employee Self Service or by contacting the DNR Office of Human Resources. • Notify SHBP/GaBreeze whenever you have a change in covered dependents (within 31 days of a Qualifying Event).

  9. Employee’s Responsibilities • If employees make an election effective in PY 2019 as a New Hire (New Hire Event) or declare a Qualifying Event resulting in a new election effective in Plan Year 2019 during the Open Enrollment window, the New Hire or Qualifying Event election will override the Open Enrollment election for Plan Year 2020. • Unless: on the confirmation page after making the New Hire or Qualifying Event election, the employee follows the automated prompt in the SHBP Enrollment Portal and makes a subsequent change to their Open Enrollment election for Plan Year 2020 (even if they previously made their Open Enrollment election prior to the New Hire or Qualifying Event.) • If an employee’s hire date occurs after November 1, 2019, the employee will only receive an Open Enrollment Window. A New Hire Window will not appear. • Employees should submit their Dependent Verification documentation for newly added dependents upon receipt of their Dependent Verification letter and no later than 45 days after enrolling their Dependent in SHBP coverage. Documents must be submitted directly to SHBP and GaBreeze.

  11. 2020 Flexible Benefits Enhancements and Changes • Employee Life Insurance – MetLife's One-Up Campaign allows all eligible active employees to enroll or increase their Employee Life coverage one level without completing a Statement of Health (SOH) form. • Spouse Life Insurance – Spouse’s age will be used to calculate Spouse Life premiums and any coverage reductions due to age. • Vision Insurance – Anthem Blue View Vision rates will increase by 2.3%. • 2020 Health Care Flexible Spending Account Limit will increase to $2,652. • Dental Insurance • Cigna DHMO network expands in the Metro-Atlanta Area. • Delta Dental rates will increase by 5.99%.

  13. 2020 Flexible Benefits Information Flexible Spending Accounts (FSAs) • Employees must re-enroll in the Health Care Flexible Spending Account (HCFSA) and Dependent Care Flexible Spending Account (DCFSA) during Open Enrollment. • If an employee has a current contribution for 2019, it will NOT automatically rollover into the new plan year. • Employees MUST make an election to continue contributions in the FSA Plans for 2020. • http://myspendingaccount.wageworks.com is the 1 st stop for research, employee inquires and to check HCFSA and DCFSA balances. • You must have a dependent under the age of 13 years old to be eligible for the Dependent Care Flexible Spending Account. • Once you enroll in a Flexible Spending Account, you may NOT cancel during the plan year. • There will still be a grace period to make claims.

  14. 2020 Flexible Benefits Information Flexible Spending Accounts (FSAs) relation to the Health Saving Account (HSA) • Employees who enroll in the High Deductible Health Plan AND enroll in a Health Savings Account (HSA) will NOT be eligible to enroll in a Health Care Flexible Spending Account (HCFSA). • If an employee does enroll in a HCFSA in error, it will be necessary for them to contact the GaBreeze Call Center and request an Appeal Form to submit to the Georgia Department of Administrative Services (DOAS). • The form will be received and reviewed by DOAS. • GaBreeze will be notified of the final determination regarding the Spending Account. • NO REFUNDS WILL BE GIVEN.

  15. 2020 Health Insurance Plan Options Health Maintenance Organization (HMO) • Anthem Blue Cross and Blue Shield • United Healthcare • Kaiser Permanente (Metro Atlanta Service Area/In-Network only) Health Reimbursement Arrangement (HRA) • Anthem Blue Cross and Blue Shield High Deductible Health Plan (HDHP) • UnitedHealthcare No changes to copays, co-insurance or deductibles

  16. Dependent Verification Services • Dependents are Not Added to coverage until the Member provides verification of their Dependent’s eligibility. (Exceptions: Birth, Adoption and Death of Dependent) • Dependents awaiting approval are placed in a PENDING status. • The required documentation must be submitted within 45 days of the event proving dependent(s) is eligible for coverage. • Documentation must be submitted by employee. SHBP will NOT accept documentation from OHR Benefits. • Members and dependents are required to have a valid Social Security Number to enroll in and continue coverage (includes children over nine months of age). • If verification documentation is approved, the dependents will be added to the member’s coverage as of the effective date per SHBP rules. • If verification documentation is denied, the dependent will not be added.

  17. Member Identification Cards Anthem and UnitedHealthcare • All SHBP Members enrolled in Anthem and UnitedHealthcare plan options will receive new Member ID cards before January 1, 2020. • Newly added Dependents pending Dependent Verification review and approval after December 15, 2019 will not receive new Member ID cards until after January 1, 2020. Kaiser Permanente (KP) • New Kaiser Permanente Members will receive new Member ID cards before January 1, 2020. • Newly added Dependents pending Dependent Verification review and approval after December 15, 2019 will not receive new Member ID cards until after January 1, 2020. Due to mailing restrictions, members may receive cards at different times.

  18. Active Employee and Medicare Reminders • SHBP will send two (2) letters to Active Members: 4 months and 2 months before the member’s or member’s Spouse’s/Dependent's 65 th birthday. • The letter will request the person turning 65 to enroll in Medicare Part B and provide proof to SHBP. • NO additional notices will be sent before retirement. Working • If Member is working, Member (and their Spouse/Dependent) will retain the active (subsidized) rate. • The Member (and their Spouse/Dependent) may provide Medicare Part B information to SHBP before their retirement. Provide Medicare To SHBP • Active Member Must Provide Medicare Part B for him/herself (and/or Spouse’s/Dependent’s) to SHBP one month prior to Active Member’s 65 th birthday and/or retirement date: Go to mySHBPga.adp.com or call SHBP Member Services at (800) 610-1863.
