Open Enrollment Open enrollment is May 1 – 15, 2020  Annual period for making decisions on health plans and flexible  spending accounts (FSA) Changes are effective July 1, 2020  Nothing is required if you are:  Not changing your health plan or membership,  Not enrolling in an FSA, and  Not participating in Premium Rewards  2

How to submit your election EmployeeDirect and select Health Benefits Direct.  Preferred submission  Employee ID is the 7 digit number on your insurance card.  Submit an Enrollment Form  Fillable form can be found on our HR Open Enrollment site  Department of Human Resource Management mailed paper copy  to employee’s address on record The deadline for submission is May 15, 2020  5

  11.  Open Enrollment Highlights Plan Year 2020-21  Employee premiums have changed  COVA Care and COVA HealthAware  Complete your health assessment on your plan website between May 1 and May 15 to earn your premium reward  Flexible Spending Accounts (FSAs)  Health FSA maximum contribution limit $2,750; increased by $50  All Health Plans  Age limits removed for autism spectrum disorder and related treatments and services  COVA Care and COVA HDHP  Age limits removed for Behavioral Health Intensive In-Home Services.  The Sydney Health App makes it easier to manage your medical and pharmacy benefits 11 11

Earn Premium Rewards for Plan Year 2020-21 For employee and/or spouse enrolled in;  COVA Care or  COVA HealthAware  Enrolled spouse must complete separate assessments  Monthly savings through payroll deduction  Employee or Spouse $17/month  Together $34/month  Complete or update your online health assessment between  May 1 and May 15 for your reward to start on July 1 Complete health assessment by the 15 th of any month to  receive a reward in six to eight weeks 13 13

Log in to your COVA Care or COVA Health Aware plan website  or app COVA Care  www.anthem.com or use the Sydney Health App  COVA HealthAware  www.covahealthaware.com or use the Aetna Health App  See Spotlight for step by step instructions  Health Assessments submitted before May 1, 2020 will not  count for the new plan year 14 14
