CHI Community Initiatives Funding • Funding is made available through statewide Medicaid Transformation Project efforts • $450,000 approved by NCACH Governing Board • Managed by a community investment process through the Coalitions for Health Improvement (CHI) • Funding is intended to: • Advance whole person health • Address the social determinants of health • Increase community – clinical collaboration

  7. Whole Person Health Our vision for whole person health is for everyone to have an opportunity to reach a state of complete physical, mental, and social well-being .

  8. Coalitions for Health Improvement • Organized in 2014 – helped bring partners together to form NCACH • Serve as community voice within NCACH’s MTP efforts • 3 CHIs – Chelan-Douglas; Grant; Okanogan • 2017 – Voting seat for each CHI established on NCACH Governing Board • 2018 – NCACH formally contracts with local health jurisdictions to provide operational support (funding and staff) for each CHI • 2018 – Community Initiatives funding and framework approved by NCACH Governing Board • 2019 – Community Initiatives Advisory Group convenes – designs community investment process • July 2019 – Funding opportunity opens to partners

  9. Project Awards Available • Small - $1 - $25,000 • Medium - $25,001 - $75,000 • Large - $75,001 - $150,000 • Very Large - $150,001 + $450,000 total – awards will be issued using a portfolio approach

  10. Project Focus Areas Note -- Other focus areas will be considered for funding, but application must clearly articulate justification for the focus area.

  11. Who Can Apply *In order to be eligible, WPCC funded-partner organizations, TCDI funded-partner organizations, and the Pathways Community HUB Lead Agency must include a minimum of one non-WPCC and/or non-TCDI partner organization, or tribal entity, as a partnering organization in the application . Additionally, this funding is not to be used to support or fund other NCACH Medicaid Transformation Project work.

  12. Lead Applicants v Partners • Lead applicant: submits application; responsible for administering funds* and delivering reporting requirements • Partner organization : Plays an active and defined role in project implementation.   13. Priority Elements Project applications have a ‘mix -and- match’ Priority Elements requirement, depending on project size.   14. “Counties served” 1. At least 15% of the target population(s) that will be impacted as outlined in the project application must reside in different counties. 2. A project only operating in Wenatchee – East Wenatchee is not considered to be multi-county.

  15. “Partnering organizations” 1. Project must include at least one other committed partner that represents a different sector than the applicant organization (e.g. healthcare and education)

  16. “Project focus areas” 1. Project must include approaches to address at least two of the project focus areas identified

Review Bodies There are 3 distinct review bodies that make up the CHI Community Initiatives Project Application review process: CHI Application Team(s) – 1 per Coalition ( 3 total ) ; responsible for reviewing LOIs and providing feedback on medium, large, and very large proposals. Reviews and scores submitted project applications Regional Review Group – 1 for the region. Reviews and scores submitted project applications Award Committee – Reviews combined scores from the CHI Application Team(s) and the Regional Review Group. Responsible for making final funding allocations to the NCACH Governing Board using a portfolio approach.

  20. PSA: Reviewers needed! Be a part of the community investment process! • We are looking for reviewers to join both the CHI Application Teams and the Regional Review groups. • Can be both an applicant and a reviewer* • Make our first funding cycle successful! *Reviewers will not be able to score their own applications, or make critical decisions about any application that presents a conflict of interest
