COVID-19 Community Engagement Funding
Funding scope of work • Funding to 179 CBOs statewide to support: o Outreach & community engagement o Contact tracing o Social services/wraparound supports • ~$25 million in CARES Act funding available through December 30, 2020 2
Who we want to reach • People of color • Faith communities • People with disabilities • Undocumented communities • People who are houseless • Farmworkers • Individuals with substance use disorder • People experiencing mental health issues • Immigrant and refugee communities • Older adults and LGBTQIA+ communities 3
Social service/wraparound support • CBO staff provide: o Wraparound: Meet short term immediate needs for isolation & quarantine (e.g., food, motel/hotel, etc.) o Safety net: Connect to established social service programs for long term needs (e.g., SNAP, housing, OHP) • Screening and assessment to determine need o Ask if person already connected to existing supports: • e.g. CCO member or DHS disability programs o Refer to existing supports • Regional Outreach Coordinator/OHP enrollment assister • DHS liaison 4
Role of CCOs • CCOs retains medical case management responsibilities for members who are in isolation or quarantine due to COVID-19. • LPHA receives reports of positive cases. LPHA/CBO social support case management is an additional benefit and resource for those members. 5
