How does PACE work? Care is highly-coordinated and managed Interdisciplinary Team (IDT) • Board Certified Geriatric Physician and NP • Nurse Case Manager • Therapists (Physical, Occupational, Recreational) • Registered Dietician • Social Workers • Chaplain • Personal Care Attendant (CNA) 4

What do PACE services include? Lab In home nursing, rehab Personal care Specialists Radiology Assisted Living Oxygen Behavioral health, Medications counseling Primary Care Emergency Transportation room care DME Subacute and residential Nursing home Adult Family Home Hearing aid, Hospital audiology, admission Outpatient rehab Eyeglasses, optometry Palliative, end of life care
What are enrollment criteria? • Age 55 + • Certified by the State as eligible for nursing home level of care • Lives in a zip code we serve • Medicaid and Medicare eligible OR able to pay a Medicaid monthly premium
What are enrollment criteria? • Living in a safe and stable environment • Willing to participate in a coordinated plan of care and receive all services from PACE and their contracted providers
How will you Benefit? • Close or extensive care coordination: wounds, DM+, anticoag, chronic pain • Benefits from ADH, community • Support for patient and family • ElderPlace Transportation • No co-pays, co-insurance • Additional benefits not covered by Medicare/Medicaid - OTCs
