Primarily for Hampshire Residents (18+) – Self service, search for local services, meet health & wellbeing aspirations using information & advice online HCC Adult Services Practitioners – to deliver strengths based approach HCC Contact Centre – to advise using authoritative sources, channel shift to online Partners – to signpost to information & advice and service directory e.g. GPs, Police, HFRS, Hospitals, CCGs, Libraries, District and Borough Councils, community and voluntary sector

  11. Community directory: What it is not X • Yellow pages • E.volve • No retail businesses (unless non-profit). What it is • Low cost or no cost community services, support and activities. National charities and helplines. • Intended as a ‘signposting’ tool. • Points to ‘source’ of information or umbrella organisation (provider, venue) wherever possible.

  13. John John is 79. He moved to the village of Crondall, in Hart, from London 10 years ago. Not long after moving, John’s wife became ill and he spent most of his time looking after her. His wife died a year ago and he has no family in the area; his three children and grandchildren live in London. He is still fairly fit and mobile for his age but does not drive any more. He does not use a computer. John used to be very sociable and he loves sport and the theatre. Having been a full-time carer for so long, John tells you that he now has no social life or friends. He only goes out to buy food for himself. He is feeling very lonely and isolated. Using Connect to Support Hampshire , what suggestions can you make to John that may help him to tackle his loneliness?

Outstanding progress – the numbers • Average hits on the site now at 10,000 a month – with over 18,000 hits in December alone! • 30% of all GP practices are engaging and using CTSH • 365,000 people reached weekly via radio advertising • CTSH App launched FIRST of its kind in the country • Joint leaflet drop with HFRS for over 65’s – 350,000 households • 60,000 pharmacy bags sent to 60 pharmacies

Outstanding progress – the partners • Working with Portsmouth City Council to onboard community directories, Southampton City Council keen to do the same • Integrated CTSH in NHS 111 service directory • Development of information on CTSH with District and Borough Council housing and wellbeing teams • Part of Hampshire Constabulary Safe at home promotion to 8,000 households • 1600 Hampshire Fire and Rescue staff engaged
