Our Goal • The goal of the Westbury Union Free School District’s Central Registration is to provide accurate, parent friendly information in a collaborative and helpful manner, resulting in the appropriate registration and placement for all students, following the consistent implementation of New York State Education Department Regulations. 2

REPORTING STRUCTURE FOR REGISTRATION AND ENROLLMENT • Registration occurs in two locations: • Office of Central Registration • Pupil Personnel Services (PPS). • Commencing on July 1, 2016 all registration will occur at the Office of Central Registration at the Dryden Street School • The District’s Building Level Coordinators are: Dale Telmer (Dryden Street), Gloria Dingwall (Park Avenue); Wanda Toledo (Drexel Avenue); Claudia Germain (Powells Lane); David Zimbler (Westbury Middle School); Manuel Arias (Westbury High School). • There is a bilingual nurse assigned to the Office of Central Registration. • The District has one data entry clerk and a Community Aide for intake at the Office of Central Registration. • There is a Security officer assigned to the Office of Central Registration. • The District has a McKinney Vento Liaison. 4

Role of the Ombudsman The Ombudsman shall be responsible for administering the District’s enrollment and academic placement processes; ensuring compliance with state and federal laws regarding school enrollment and academic placement; ensuring that the implementation of the District’s enrollment and academic procedures is fair, equitable, and communicating with students, parents, and staff about issues relating to the District’s enrollment and academic placement policies and the implementation.

  6. Our Purpose The Office of Central Registration is responsible for the registration of each child entering the Westbury Union Free School District. It is the responsibility of the parent/person in parental relation to show adequate documentation proving that both the parent/person in parental relation and child are legal residents of the school district. 6

Registration Disclosure All students over five and under twenty-one years of age, and regardless of English language proficiency, are entitled to attend a daytime academic program at a school district that allows such students to obtain credit toward a Regents diploma. Students who indicate an interest in enrolling in a non-Regents bearing program shall be provided with information regarding his/her options and will be required to complete an Parent and Student Consent Form. Such disclosure shall be made by the district prior to the student’s choice of academic placement. The registration coordinator and individuals under his/her supervision will be responsible for such disclosure. 7

Registration Disclosure, Cont’d Procedure: (1) If a student or parent expresses an interest in an alternative program, and if the District is then offering an alternative program, the staff member processing the enrollment application will provide the following statement in the parent’s and student’s primary language: All students over five and under twenty-one years of age, and regardless of English language proficiency, are entitled to attend a daytime academic program at a school district that allows such students to obtain credit toward a Regents diploma. (2) Under no circumstances shall a District employee require or steer a student into an alternative educational program. (3) If a new student indicates an interest in enrolling in an alternative educational program, a District employee may provide information to the student regarding his/her options and the District employee will notify the parent and student that they must complete Parent and Student Consent Forms. (4) Provide the parent and student with details of any alternative program then being offered by the District. Written details will be provided to you for this purpose.   9. Registration Disclosure, Cont’d (5) If necessary, a qualified interpreter will be used for all disclosures related to an alternative program. The staff member will arrange for a qualified interpreter to be present for the disclosure. Contact Juanita Sherwood at (516) 874-1658 to obtain an interpreter. (6) If, after the foregoing disclosures, the parent and student wish to enroll in an alternative program, the parent and student must sign a Parent and Student Consent Form (see the following pages). Forms will be provided in English, Spanish and Haitian Creole. If the primary language of the parent or student is another language, a qualified interpreter must be obtained and must countersign the Consent Form. (7) Enrollment in an alternative educational program is voluntary and is at the student’s and parent’s option, and only after the above disclosures have been made and Consent Forms signed. 9

  10. Parent Consent Form I am a resident of the Westbury Union Free School District. My child, _______________, is ___ years of age. According to New York State Law, he/she is entitled to attend the schools of the Westbury Union Free School District until he/she graduates or reaches the age of 21, whichever occurs first. Because my child has surpassed the compulsory age for school attendance in New York, he/she is not required to attend school. At this time, I am waiving my child’s entitlement to attend school in the Westbury Union Free School District. I understand that my child may enroll in the District at a later date, prior to reaching the age of 21. The District’s representative has explained to me all of the options and rights associated with enrollment in the Westbury Union Free School District. My child will _____ will not ______ be enrolling in the Westbury Union Free School District’s Adult Learning Center for the purposes of earning his/her general equivalency diploma. ___________________________ __________________ Signature Date If a student / guardian speaks a language other than English / Spanish/ Haitian-Creole, a qualified interpreter in the individuals home language must sign the consent form to verify the content. 10
