SHI P Grant Program  Administered through the U.S. Department of Health and Human Services, Health Resources and Services Administration, Federal Office of Rural Health Policy  Purpose  Implement quality and operational improvement efforts 5

Who is Eligible?  Eligible hospitals  Located in the U.S. and its territories  49 beds or less  Non-federal hospitals  May be for-profit, not-for-profit, or tribal  Critical access hospitals are eligible 36 Critical Access Hospitals 15 Small Rural Designation 6

SHI P Funding Priorities Type of Hospital HCAHPS I CD-10 Critical Access Hospitals Required Required Non-Critical Access Hospitals Required Required • If hospitals have fully implemented HCAHPS and ICD-10, they can choose other options from the SHIP purchasing menu • If the purchasing menu has been exhausted, hospitals may select other projects, contingent upon the approval of the designated SHIP coordinator 11
