VC-Pact History Funded by Lucile Packard Foundation for Children’s Health in Jan 2015 as part of the 5 C’s Project Goal: Establish a sustainable, family-centered, coordinated coalition with existing resources and collaborative relationships to improve Ventura County’s local systems of care for children with special health care needs. 2

Lessons Learned • Delays are often caused by misconceptions about services provided by an agency, their referral process, and eligibility criteria • Access to services is limited by agencies funding requirements/scope of work and limitations • Parents continue to be the primary care coordinators for their children and navigating the “system” is difficult. Finding a lead agency to “coordinate” needs is essential. • Listen to the family and what their needs are. Even if your agency offers great services, if it is not a priority to the family, follow through is difficult. 6
