Case and Contact Management • Identification of a probable or confirmed case triggers an investigation to assess: – Potential exposures – Potential transmission • Allow for interventions to prevent further transmission
Public Health Framework • ‘Containment’: contain and eliminate the spread of virus • ‘Mitigation & Response’: to decrease morbidity and mortality caused by COVID-19 • ‘Recovery’: to routinize response to this virus across Ontario Containment Mitigation & Response Recovery
Who is a Case? • Individual with disease – AKA the patient • Probable vs Confirmed
Who is a Contact? • Someone with who is at risk of developing the disease because of exposure to the case • Not all contacts are equal – Close contacts have had a high-risk exposure, such as household contacts, HCWs without PPE, etc.
Three Key Questions 1. Where did they get it? 2. Who can they spread it to? 3. What can we do to slow the spread of the disease?
Question #1 Incubation Period • Time from exposure to the causative agent until the first symptoms develop • Unique characteristic for each disease • Estimated incubation period for COVID-19 is 2-14 days, with an average of 5 days
Question #2 Period of Communicability • Time during which an infectious agent may be transferred directly or indirectly from an infected person to another • Estimate period of communicability for COVID-19 is currently assumed to be 48 hours before the onset of symptoms to 14 days after
Question #3 Isolation vs. Quarantine • Isolation – Isolation separates sick people with a contagious disease from people who are not sick • Quarantine – Quarantine separates and restricts the movement of people who were exposed to a contagious disease to see if they become sick
Question #3 Who needs to self-isolate? • Symptomatic people! • Any individual with COVID-19-like symptoms should self-isolate
Question #3 Discharging from self-isolation • Must be no longer contagious • Different discharge criteria exist for different circumstances – See guidance document
Question #3 Who needs to quarantine for COVID-19? • Asymptomatic people! • Any individual who has a high-risk exposure – Close-contact to a suspected or confirmed COVID-19 case – Recent travel to an international destination
Question #3 Discharging from quarantine • Individuals who have a high-risk exposure to a suspected or confirmed COVID-19 case should quarantine for 14 days from the exposure
