  3. T: Tool • Getting to the Heart of the Matter Tool used, if applicable • Example: T: magic wand. Mom wishes she had enough milk to exclusively breastfeed baby.

  4. G: Goals • Personal goals or areas identified by participant that they plan to work on • Example: Mom plans to latch infant more often to her breast, aiming for at least 8xday. She will try to latch baby at signs of early hunger cues instead of waiting until her baby is crying.

  7. F: Follow-up • Any information that the staff person has identified as areas to follow-up with at subsequent visits. • Note: The TGIF documentation shall be tailored per individual. Documenting the identical note in each individual care plan in a family is not acceptable.

  8. Why are notes important? • USDA requirement • Continuum of care • Completes documentation of nutrition assessment • If there is no note at certification, the certification is no longer active (not for PCs) • Protect yourself

Educated on supply and demand, enough wet/dirty diapers. – Mom reports doctor was concerned with infants weight at last visit, but she is not worried. Encouraged mom to follow up with pediatrician, offered to weight infant today. – Mom doesn’t know how many times per day she BFs, but sometimes gives the pacifier so she can finish errands. Discussed BF 8-12xday, encouraged mom to keep track for a few days. Refreshed on hunger and fullness cues. Gave examples to mom. – No referrals made at this time, mom is happy with BF Referred mom to IBCLC and Pediatrician. Offered BF Hotline# • F: IBCLC/RD will F/U in one month ASAP
