Screening, Assessment and Support Services Ø Statewide Program that provides services for children and adolescents who are in crisis and may be in need of intensive services or inpatient hospitalization Ø Cooperative Partnership that created a single statewide system whose care requires public funding from one of these three agencies: ü Department of Children and Family Services (DCFS) ü Healthcare and Family Services (HFS) ü Department of Human Services (DHS) Who can receive SASS Services? Ø Any youth that is presenting in a psychiatric crisis AND Ø Age and Financial eligibility ü Children under the age of 18 seeking public funding for psychiatric services through DHS ü Children and adolescents under the age of 21 enrolled in HFS’ medical programs ü Any person for whom DCFS has legal responsibility 1

Ø Risk of harm to self Ø Youth indicating they have been having thoughts of suicide Ø Passive: “It would be better if I weren’t here”, “No one would miss me if I was gone” Ø Direct: “I want to die”, “I’ve been thinking about killing myself” Ø Discussion of plan and means to carry out the plan Ø Have they thought of how they would do it, when they would do it, and have they taken steps to follow through Ø Self harm behaviors Ø Cutting, burning, or other ways to “harm”, not complete suicide What is considered a crisis? Ø Risk of harm to others Ø Youth indicating they have thoughts to hurt others Ø Passive: “They’ll be sorry”, “He/she will pay for that” Ø Direct: “I’m going to beat him up”, “I’m going to kill him” Ø Discussion of plan and means to carry out the plan Ø Specific time/way/manner they will hurt someone Ø Aggressive behaviors Ø Intentional vs. unintentional aggression Ø Discussion of past history of aggression What is considered a crisis? Ø Other behavioral health crisis Ø Psychosis Ø Decompensation in functioning Ø High risk behaviors that may put self or others at risk of harm 2
10/15/17 How are services accessed when imminent risk is suspected? CRISIS AND REFERRAL ENTRY SERVICES (CARES) 800-345-9049 Ø Single point of entry hotline for all SASS calls throughout the state Ø CARES staff complete the following: Ø A brief acuity assessment to determine need Ø Assessment for age and financial assessment criteria Ø Referral to the SASS provider where the youth is presenting in crisis As the referral source, be prepared to provide: Ø Information that may be asked by CARES: Ø Demographic information (Name, DOB, Address, Phone, etc) Ø Location of the crisis Ø Immediate threat to safety Ø Presenting problem Ø Duration of symptoms Ø Parent/guardian information and availability Ø Please make sure that parents are on site if at all possible to ensure the SASS screener can complete disposition (safety plan, hospitalization). If not available to be on site, please inform them of the call to CARES and ask they be available by phone to consult with screener When to Call 911 instead of CARES Immediate safety risk: Ø Youth is in crisis that requires immediate response Ø Unable to ensure safety of youth in crisis for the 90 minute window for screener to respond on site to where the crisis is occurring Ø Unable to monitor (have eyes on the youth) until screener arrives Ø Youth has made an attempt or is in the process of harmful behavior Ø Environment does not allow for a safe screening Ø Caller feels that the situation is unsafe and requires police involvement 3

SCREENING Ø 90 minute response from the time of call from CARES to have a screener on site where the youth is presenting in crisis Ø SASS Screening and Risk Assessment to determine if there is a need for inpatient hospitalization or whether they can contract for safety Ø Screening completed by an Emergency Services Counselor Ø Full Risk Assessment to determine appropriate least restrictive level of care Ø Collaboration with referral source, family, youth and any other professionals involved in care to determine best plan of action What services are offered? SCREENING Ø Hospitalization ü SASS screener will work with parent/guardian to determine the most appropriate hospital in the area ü Facilitate admission ü Coordinate transportation Ø Community Stabilization ü Develop a comprehensive safety plan to aid in stabilization of symptoms in the home ü Provide follow up within 48 hours of initial evaluation What services are offered? SUPPORT SERVICES Ø All clients referred through the CARES hotline are eligible for up to 90 days of community based services that include: ü 24-Hour crisis intervention ü Assessment and Treatment Planning ü Collaboration and discharge planning with inpatient facility (if applicable) ü Care coordination with any existing mental health providers ü Care coordination with other family supports ü Intensive individual and family therapy/counseling ü Psychiatric evaluations and medication monitoring ü Case management ü Linkage to long term services as appropriate ü Parent support group 4

Family Resource Developer The SASS program has a Family Resource Developer on staff to provide support to families and assist them in achieving the best possible outcomes for their child. Ø Experience as a parent, caregiver, or guardian who has gone through a crisis with their own child Ø Assists families in identifying resources in their community based on needs of the child and family Ø Answer questions and provide support Frequently Asked Questions?
