Minimum Qualifications  Each Respondent is required to have at least one of the following within the past 5 years:  Two (2) or more continuous years or  Two (2) or more continuous years of senior or executive management  Three (3) or more continuous years - management staff member or executive officer

  8. Contract Term  Three (3) years  With two (2) one-year renewal options

  9. Response Guarantee  $2,500  Must be a cashier’s check payable, without condition or restrictive endorsement, to the City of Phoenix

  10. Response Limitation  One (1) per Respondent or Business and from any affiliates  Multiple Responses will be non-responsive

  11. Established Business  Arizona Corporation Commission (ACC) Registration  Prior to the award of the lease

  12. Airport Security  All individuals assigned to work at the airports must:  Pass fingerprint-based Criminal History Records Check  Federally-Mandated Security Threat Assessment  Aviation-issued security credentials or badge

  13. Scope of Work 1. 37 Machines – 13 locations PHX, DVT & GYR  Locations and machines based on current placement  Creative options are encouraged  Concepts should meet employee needs

  14. SCOPE OF WORK 2. Flexibility to increase/decrease machines or locations  Provide creativity on type of machines

  15. Scope of Work 3. Service offerings  Cold Beverage (non-alcoholic)  Snacks and convenience meal items  Cold Foods (freezer meals, ice cream)  Prohibited items: medication, alcohol, tobacco, electronics, gambling items

  16. Scope of Work 4. Micro Market Opportunity Aviation Department Office Building  Building hours: M-F, 8:00 a.m. - 5:00 p.m.  Space for the equivalent of 5 machines  Potential for camera/data lines
