• Founding Information – Founded in 1973, the Puerto Rican Cultural Center is a community-based, grassroots, educational, health and cultural service organization founded on the principles of self- determination, self-actualization and self- sufficiency that is activist-oriented. – PRCC addresses fundamental needs in the Puerto Rican/Latino communities throughout Chicago with emphasis in Humboldt Park, Logan Square and Hermosa Park – Motto: “ Live and help to live .”
Ryan White Services Overview Funding for programming began in 2015 Program services include providing on- and off-site comprehensive psychosocial and substance use group and individual sessions. Participants are provided with ongoing support and tools needed to maintain a healthy substance-free lifestyle. Other relevant HIV funding received: Ryan White Part B, ADAP, SAMHSA – Minority AIDS Initiative Current service area includes Humboldt Park, Lawndale and Calumet Park but also includes all of Chicago areas. Demographics include African American and Latinx
PDSA Plan: Improve Client Attendance and adherence to treatment goals Do: Increase communication with case managers to coordinate reminders and collaborate on goals Communicate with Charlotte to obtain case contact list for current clients manager Track with weekly reports dating from 8/1/17 Study: Having on file case managers, follow up with case manager assigned, work collectively Act: Client adherence to medication
