What we are going to do in this session We’re going to look at some of the outcomes we need to achieve. Some of these are set nationally and some are local outcomes we have been asked to focus on by organisations and communities in North East Lincolnshire . Some of these outcomes focus on care and treatment, others focus on preventing ill health. We need to get the balance right in where we direct our resources. That’s what we want to discuss with you today and get your thoughts.
The NELives Framework provides the context and background for the health and care outcomes we want to achieve as part of our approach to delivering the NHS Long Term Plan
Vitality and Health – our aims We want people to be informed, capable of living independent lives, self-supporting and resilient in maintaining or improving their own health. We want safe, quality services that prevent ill health where possible, support, maintain and restore people back to the best of health for them or support them with dignity right through to the end of their lives
5 Things we have to do • The NHS Long Term Plan sets out a vision for the next ten years • Alongside other organisations in the Yorkshire and Humber Integrated Care System (Humber Coast and Vale STP), we will be contributing to the five year strategy • This will need to be produced by October 19 and will set out work to take us to the half way mark in delivering the Long Term Plan
Anticipated Financial Aims • Financial balance across the system (commissioners and providers) • Integrated financial planning across the NEL Health & Wellbeing system resulting in: (a) the optimum value for money (Best Use of the NEL pound) (b) clear understanding of the ”place” costs
National Outcomes – treatment • Primary Care – offer accessible primary care which deals with heath needs as swiftly and effectively as possible, including providing access to other services and types of care where needed. • Cancer – reduce preventable deaths by increasing screening uptake and early diagnosis • Community urgent care – increase access to the right care at the right time • People are supported to return home promptly following a hospital stay • Planned Care – enable timely access to specialist care such as planned operations in order to return to optimal health as soon as possible
National Outcomes - prevention • Maternity/Women and Children – deliver the best possible start in life for all children, starting with Better Births and undertaking targeted work around vulnerable children • Prevention – reduce preventable periods of ill health by promoting, encouraging and enabling health seeking behaviours for our population • Medicines Optimisation – ensuring deployment of the most cost-effective and clinically safe medicines for each condition • Adult Social Care – enable the optimal length of independent living for each individual and subsequently enable appropriate care when it is needed • End of Life – increase the percentage of people dying in their place of choice
Local Outcomes – treatment and prevention • Focus on multi-disciplinary approach to frail patients • Substance misuse – reduce dependency on alcohol and drugs for specific identified cohorts of our population. • Mental Health – reduce self-harm admissions to hospital through improved crisis services. • Reduce the 20 year gap in life expectancy due to physical health issues experienced by people with mental health issues

Projects that will help us deliver our aims • Focussed work on screening and improving cancer early diagnosis for lung and colorectal cancers • Support at home – supporting people to stay at home or return home promptly after episodes of ill health • Reshape service provision for Long Term conditions including Community Cardiology • Special Educational Needs and Disability (SEND) - Implement action plan • Integrated Urgent Care - Work with PCNs to establish an enhanced community urgent care response • Baseline assessment of current End of Life provision to identify gaps and areas for action • Implement Advice and guidance with secondary care
Projects that will help us deliver our aims • Outpatient Transformation including - Daycase to Outpatient procedures, Digital solutions, Integrated specialist services • Improve access to physical health checks for people with mental health issues • Out of Area specialist rehab reduction • Harmonise medicines formularies between primary and secondary care • Reshaping Primary Care around PCNs to include community multi-disciplinary teams • Social Prescribing • Self care through digital solutions (whilst maintaining options for telephone and face to face) • Focus on multi-disciplinary approach to frail patients with the aim to reduce admissions from this cohort by 20%
Where we’re up to • Analysed the aims set out in the NHS Long Term Plan • Drafted aims and projects which will help us contribute to the national delivery of the Long Term Plan • Working with the Humber Coast and Vale Partnership (STP) to formulate the collaborative plan • More detailed implementation is being worked up • The timeline for preparation in 19/20 and implementation in 20/21 is on track
