The National Diabetes Audit (NDA) is commissioned by the Healthcare Quality Improvement Partnership (HQIP) as part of the National Clinical Audit and Patient Outcomes Programme (NCAPOP) following advice to the Department of Health from the National Advisory Group on Clinical Audit and Enquiries (NAGCAE). Back ground There is considerable variation in outcome of foot disease The reasons are not fully understood but include I. Race II. Access to effective primary care III. Professional training and expertise Data The NDFA is a new module of the pre-existing National Diabetes Audit- which collates data from individuals using automated downloads from GP databased. It is linked to anonymised data on over 80% of the diabetes population in England and Wales The aim of the NDFA is to identify all newly presenting episodes of foot ulcer and to explore factors linked with differences in outcome (healing amputation survival) It comprises audit of both the structures and the process of care
Data Collection and Consent forms
Principles of data collection Capture RELIABLE data on all newly presenting ulcers Progress The audit started in July 2014 170 centres have registered with the scheme o Hospital – diabetes,/ vascular services o In-patient and out-patient referrals o Podiatry – community, community hospital o General practice Submission deadlines The first deadline for completing data entry will be 31 July 2015 . o For episodes with first assessment on or before 10 April 2015 , the data from Section A of the form and the 12 week outcome information should be entered by 31 July 2015. The National Diabetes Foot care Audit is a continuous data collection continue to consent, collect and enter patient records.
Structure Audit September/ October 2015 CCG will be approached to provide information from those responsible for organising foot care services, commissioning leads will be contacted via an electronic form. Questions asked will include : I. A training scheme for all people involved for screening for foot risk in people with diabetes, II. A pathway for referring people at increased risk for specialist preventive care III. A pathway for rapid referral of all people with a new or deteriorating ulcer for urgent expert assessment. Reports Each participating service will receive reports showing comparisons between services for: o Number of ulcers treated o Case-mix of ulcers treated o Outcomes of ulcers treated (standardised for case mix) o Standardised Healing Ratio o Admissions o Amputations o Deaths
Publication Dates Publish report from first year of data collection (July 2014 to 2015) in March 2016 Publish Patient & Public version of report by June 2016
